右室流出道室性早搏患者的窦性心率震荡变化

摘　　要：目的观察右室流出道室性早搏患者窦性心率震荡(HRT)的变化并探讨其临床意义。方法52例右室流出道室性早搏患者按室早数目分为两组:A组(室早≥1000/24h)和B组(室早<1000/24h),比较两组间的HRT参数震荡初始(TO)和震荡斜率(TS)。结果右室流出道室性早搏患者HRT异常的发生率为23.08%,且A组患者中异常HRT的发生率明显高于B组;与B组相比,A组患者的TO值明显增大,TS值降低(p均<0.05);HRT与室早数目呈明显正相关(p<0.01),TS与室早数目呈明显负相关(p<0.01)。结论频发右室流出道室性早搏(室早≥1000/24h)患者的窦性心率震荡现象减弱,且HRT与室性早搏数目明显相关。HRT是一种较好的检测心脏自主神经功能的新技术。Objective To explore the change of heart rate turbulence（HRT）in patients with ventricular prema- ture beat （VPB）in the right ventricular outflow tract and its clinical significance. Methods 52 patients with VPB in the right ventficular outflow tract were divided into two groups：A group （VPB ≥1000/24h）and B group （VBP 〈 1000/24h）. Two HRT parameters： turbulence onset （TO）and turbulence slope （TS） were measured using digitized 24h Hoher recordings. Results The incidence of abnormal HRT in the patients with VPB in the right ventricular out- flow tract was 23.08%.The value of the TO was significantly higher and the value of the TS was significantly lower in the A group than those in the B group. Significant positive correlation between the TO and the number of VPBs, negative correlation between the TS and the number of VPBs,were observed（p 〈 0.01 ）. Conclusion HRT was weak- er in the patients with frequent VPB （ t〉 1000/24h）in the right ventricular outflow tract and correlate strongly with the number of VPB. HRT should be considered as a better way for determination of cardiac autonomic nervous function.
