摘 要:目的探讨E-Tracking技术检测2型糖尿病(T2DM)患者颈动脉弹性的应用价值。方法根据颈动脉超声的检查结果,将62例T2DM患者分为两组:A组(颈动脉内中膜增厚和/或斑块存在,32例),B组(颈动脉内中膜无增厚亦无斑块存在,30例)。另选35例正常人作为对照组(C组)。用E-Tracking技术自动跟踪颈总动脉壁的运动,检测收缩期颈总动脉管径(Ds)、舒张期颈总动脉管径(Dd)、压力-应变弹性系数(Ep)、硬化参数(β)、及顺应性(AC)、脉搏波传导速度(PWVβ)和增大指数(AI)。结果A组和B组患者的Ep,β,PWVβ,AI较C组显著增加,AC较C组显著减低。与B组比较,A组的Ep,PWVβ显著增高,AC显著减低。另外,A组和B组患者的Ds和Dd较C组显著增宽。结论在颈动脉出现内中膜增厚或斑块之前,T2DM患者动脉弹性已经发生了变化。E-Tracking技术可用来检查动脉粥样硬化早期的动脉弹性改变。Objective To explore the value of E-Tracking technology in evaluating the elasticity of the carotid artery in type 2 diabetes mellitus (T2DM) patients. Methods A total of 62 cases of T2DM patients were divided into two groups according to the results of B-mode ultrasonography of the carotid artery: group A (with intima-media thickness increasing and/or with plaques of the carotid artery, 32 cases) and group B (without intima-media thickness increasing and plaques of the carotid artery, 30 cases); and 35 cases of normal subjects were selected as group C (control group). E-Tracking technology was used to follow the movement tracks of the common carotid arterial wall, and the following parameters were assessed respectively: systolic diameter of common carotid artery (Ds), diastolic diameter of common carotid artery (Dd), pressure-strain elastic modulus (Ep), stiffness parameter (β), arterial compliance (AC), pulse wave conducting velocity (PWVβ) and augment index (AI). Results The parameters of Ep, β, PWVβ, AI in group A and group B were all significantly higher than those of group C, and AC was significantly lower than that of group C. Compared with group B, the parameters of Ep, PWVβ in group A were all significantly higher, and AC was significantly lower. Both Ds and Dd were significantly wider in group A and group B than those of in group C. Conclusion The elasticity of the carotid artery in T2DM patients has changed before the existing of intima-media thickness increasing or plaques. E-Tracking technology can be used to detect the early elasticity changes of arteriosclerosis.
