摘  要:目的应用血管回声跟踪技术评价二甲双胍对2型糖尿病患者血管内皮功能的影响。方法初诊2型糖尿病患者48例,使用高频线阵探头,实时跟踪描记右侧肱动脉的运动轨迹,分别于二甲双胍治疗前后,测量血管收缩期最大内径(Ds)、舒张期最小内径(Dd)、僵硬度(β)、弹性模量(Eρ)、顺应性(AC)及脉搏波传导速度(PWVβ)等数项指标,评价二甲双胍对血管内皮功能的影响。结果服用二甲双胍3个月后,血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、空腹血糖(FBG)、糖化血红蛋白(HbA1c)较治疗前明显减低(P〈0.01~0.001);高密度脂蛋白(HDL)、体质指数(BMI)、心率(HR)、收缩压(SBP)、舒张压(DBP)、Ds及Dd治疗前后差异均无统计学意义(P〉0.05)。β,Eρ,PWVβ治疗后较治疗前明显减低(P〈0.001),AC测值明显增高(P〈0.001)。结论血管回声跟踪技术能较好地评价二甲双胍对2型糖尿病患者受损血管内皮功能的改善作用。Objective To evaluate the effects of metformin on vascular endothelial function in type 2 diabetes mellitus by ultrasonic E-Tracking imaging technology. Methods Forty-eight newly diagnosed type 2 diabetes mellitus were selected. With a high frequency linear array probe, E-Tracking imaging technology was performed to follow the movement tracks of the right brachial arterial wall, parameters including internal systolic diameter (Ds), internal diastolic diameter (Dd), stiffness parameters (β), pressure-strain elastic modulus (Eρ), arterial compliance (AC), pulse wave velocity (PWVβ) and so on were assessed before and after three months treatment with metformin to evaluate the effects of metformin on vascular endothelial function in type 2 diabetes mellitus. Results After three months treatment with metformin, total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL), fasting blood glucose (FBG), and glycosylated hemoglobin (HbA1c) sigificantly decreased than before (P〈0.01-0.001);treatment effects on high density lipoprotein(HDL), body mass index(BMI), heart rate(HR), systolic blood pressure(SBP), diastolic blood pressure(DBP), Ds and Dd did not have statistical significance (P〉0.05).The parameters of B, Ep and PWVB decreased conspicuously (P〈0.001), and AC was significantly higher after treatment (P〈0.001).Conclusion E-Tracking imaging technology can be used to evaluate the effects of metformin on vascular endothelial function in type 2 diabetes mellitus.
