摘 要:目的探讨核因子-κB(NF-κB)在儿童原发性肾病综合征(PNS)发病机制中的意义及黄芪对PNS患儿外周血单个核细胞(PBMC)NF-κB活性的影响。方法选取25例PNS患儿(PNS组)和20例健康儿童(健康对照组)作为研究对象。抽取所有研究对象5mL静脉血并提取PBMC,分别加入NF-κB刺激剂IL-1β、NF-κB抑制剂对甲苯磺酰-L-苯丙氨酸氯甲基酮(TPCK)及黄芪注射液。经培养后离心,留细胞沉渣提取核蛋白,采用ELISA检测其NF-κB活性。结果PNS组PBMC的NF-κB活性较健康对照组明显增高(P<0.05)。在常规培养状态下黄芪明显降低PNS组NF-κB活性(P<0.05),但不影响健康对照组NF-κB活性(P>0.05)。经IL-1β刺激后,PNS组和健康对照组NF-κB活性均增高,黄芪可降低IL-1作用后的NF-κB活性(P<0.05);经TPCK抑制后,PNS组和健康对照组NF-κB活性均降低,黄芪提高TPCK作用后的NF-κB活性(P<0.05);黄芪对PNS组PBMCNF-κB活性的影响较健康对照组明显(P<0.05)。结论PNS患儿PBMC的NF-κB活性上调,黄芪降低PNS患儿PBMC的NF-κB活性,NF-κB可能是黄芪治疗PNS的作用环节之一,黄芪对PBMC的NF-κB信号途径具有双向调节作用。Objective To investigate the pathogenesis of peripheral blood mononuclear cells (PBMC) nuclear factor kappa -κB ( NF -κB) in children with primary nephritic syndrome (PNS) and the effect of astragalus on the activity of NF -κB. Methods Twenty - five children with PNS and 20 normal children were studied. Isolated PBMC were separated from 5 mL venous blood in asepsis condition. NF -κB stimulator, NF -κB inhabitor and astragalus were added into the different tubes of PBMC, respectively. The nuclear protein was extracted from the pellets and the optical density(A ) values of nuclear protein was measured by enzyme -linked immunosorbent assay (ELISA). Results The activity of PBMC NF-κB in PNS group was higher than that in normal group ( P 〈 0. 05 ). In the normal culture conditions, astragalus could decrease the activity of NF -κB in children with PNS ( P 〈 0. 05 ) , but didn't affect that in normal children ( P 〉 0. 05 ). Astragalus could decrease the activity of PBMC NF -κB which had been stimulated by interleukin - 1 β ( IL - 1β) ( P 〈 0.05 ) ; and astragalus could make the activity of NF -κB elevated which had been inhibited by N - tosyl - phenylalanine chlormethyl - ketone ( TPCK ) ( P 〈 0. 05 ). A significant effect of astragalus on the activity of PBMC NF -κB in children with PNS was observed compared with that of normal group( P 〈 0.05 ). Condusions In children with PNS, the activity of PBMC NF -κB was up - regulated. Astragalus can decrease the activity of NF -κB in children with PNS. Regulation of NF -κB may be one of mechanisms that astragalus can treat PNS. Astragalus can regulate semaphoric approach of PBMC NF-κB bilaterally.
