单侧椎板开窗髓核摘除术治疗中央型椎间盘突出症

摘　　要：[目的]探讨单侧椎板间开窗髓核摘除术治疗中央型腰椎间盘突出症的适应证及疗效。[方法]对自2004年1月～2006年12月收治的66例确诊的中央型腰椎间盘突出症患者行单侧椎板间开窗髓核摘除术。[结果]全部病例得到3～36个月的随访(平均17个月)。疗效按照NaKai评定标准,其中优48例,良13例,可4例,差1例。优良率92.4%。疗效差的1例分析原因可能因为术中过度牵拉神经根所致,予以卧床休息2周,并加用激素、神经营养药物等,2周后缓解。本组病例无意外损伤及感染等病例发生。[结论]单侧椎板间开窗髓核摘除术是治疗中央型腰椎间盘突出症的有效方法,临床效果满意。[ Objective ] To evaluate the indication and clinical results of treatment of central disk protrusion by applying uni-vertebral plate windowing disceetomy. [ Methods] 66 cases were diagnosed positively from January 2004 to December 2006. Uni-vertebral plate windowing discectomy was applied in all cases. [Results] All cases were followed up 3 to 36 months （mean 17 months）. Clinical assessment was based on NaKai standard. Excellent was 48 cases, good was 13 cases, fair was 4 cases and poor was 1 which was concerned that nerve root over dragged in the operation might be the reason and symptom relieved after staying in bed for two weeks and using the drugs of hormones and neumtmphic drug. No infection and other complications were found in the group. [Conclusion] Uni-vertebral plate windowing lumbar disceetomy is an ideal method for treatment of central disk protrusion and clinical outcome is satisfied.
