摘 要:背景文献报告肝阳上亢证是高血压最常见证型,中医认为肝阳上亢证是肝肾阴虚,阴不制阳而致肝阳偏亢,因而传统治疗着眼于平肝潜阳法。目的观察天麻、钩藤、石决明、牡蛎、牛膝混合剂(平肝潜阳法)对自发性高血压大鼠(SHR)的降压作用及对血管重构的影响并探讨其可能的机制。方法 SHR 随机分为3组,未治疗SHR 组(n=10,等量生理盐水)、依那普利组[n=10,1.75 mg/(100 g·d)]和平肝潜阳法组[n=10,0.1 g/(100 g·d)]。SD 大鼠(n=10)作为正常对照。观察各组大鼠收缩压、主动脉中膜厚度(MT)、腔内径(LD)、MT/LD 以及中膜血管平滑肌细胞(VSMC)超微结构变化,运用 RT-PCR 和 Western Blot 方法检测管壁脂联素的表达。结果与正常对照大鼠相比,未治疗 SHR 组收缩压明显上升,主动脉 MT、MT/LD 均显著增大,主动脉壁 VSMC 表型变化明显,动脉壁脂联素表达显著减弱。与未治疗 SHR 相比,平肝潜阳法和依那普利使收缩压明显下降,主动脉 MT、中膜厚度与腔内径比值( MT/LD)均显著下降,可以逆转主动脉壁 VSMC 表型变化,动脉壁脂联素 mRNA 及蛋白表达显著增强。结论平肝潜阳法不仅能够有效地降低 SHR 血压,而且能够对动脉血管重构具有明显地逆转作用,其机制可能与上调血管脂联素表达有关。Background Excessive "Liver-Yang" is the common symptoms in hypertension. Traditional Chinese medicine reasons that "supressing Yang" and "calming liver" may decrease the high blood pressure and ameliorate the damaged target organs. Objective To study the effect of Chinese traditional treatment "calming liver" and "suppressing Yang "solution mixture of Gastrodia, procumbens, concha haliotidis, aehyranthes bidentata in spontan- eous hypertension rats. Methods Spontaneous hypertension rats were randomlized as followings: enalapril ( 3.5 ng/200 g · d, n=10), "calming liver" and "suppressing Yang" mixture solution( 1 mL/100 g · d, n= 10) with SD rats(n=10) and untreated SHR(n=10) served as control. SBP and morphological changes in aortic wall: medial thfckeness(MT), luminal diameter (LD), and the ratio of MT to LI) were determined and uhrastructral changes in VSMC were investigated. RT-PCR and Western blot were used to determine the expressions of adiponectin in the vessel wall. Results Both "calming liver" and "suppressing Yang" treatment and enalapril decreased SBP, aorta MT and MT/LD, and increased the adiponectin expression in aortic wall. The effect of suppressing Yang treatment on SBP, aortic MT/LD, and adiponectin expression in vascular vessel are comparable to that by enalapril. Conclusion The calming the liver and suppressing Yang treatment decreased blood pressure, and effectively attenua ted arterial vascular remodeling and up regulation of adiponectin expression in vascular vessels.
