乳胶增强免疫抑制法测定HbAlc的实验研究

摘  要:目的评价全自动生化分析仪乳胶增强免疫抑制法测定糖化血红蛋白(HbAlc)的可靠性,并与常规方法(微柱法)作相关比较。方法采用乳胶增强免疫抑制法测定糖化血红蛋白,同时用高铁血红蛋白法测定总血红蛋白,然后计算出糖化血红蛋白的百分比。结果该方法的批内和批间CV<4.3%,在3.1%~20.0%范围内线性良好。与BioSystems(Spain)公司的微柱法试剂进行方法学比较,差异无显著性(n=220,Y=1.01X-0.32,r=0.9756,P>0.05),两者有很好的相关性。且高浓度的尿素、血脂、胆红素和血糖对该法测定无干扰,红细胞压积比从30%到100%也不影响HbAlc的测定。结论按照美国糖尿病学会(CLIA,88)要求的允许误差标准,乳胶增强免疫抑制法测定糖化血红蛋白的结果符合临床对糖尿病患者的治疗监测要求,且该方法能进行自动化分析,具有快速、准确、简便等优点,可广泛推广使用。Objective To evaluate the credibility of the HbAlc's measurement by latex - sticky immunity repression method used the full - automatic bio - chemical analytic instrument, and maked the related comparison with the normal common method(affinity chromatography). Methods Determined the percentage of glycohemoglobin in blood by latex - sticky immunity repression method, as the same time measured total Hb by colorimetric method, and then calculated the percentage of HbAlc. Results The within - assay and total imprecision CV were less than 4. 3%, and it was good between 3.1% and 20. 0%. And as compared with the affinity chromatography reagent of BioSystems (Spain)company,there was not different(n =220 Y = 1.01X -0. 32,r =0. 9756, P 〉0. 05). The relativity of two methods was very good. This method wasnt influenced by high concentration of urea, hyperlipemia, icteric and blood glucose and hematocrit. Conclusion The results of HbAlc that were determined by latex - sticky immunity repression method were agreed with the request of the treatment and monitor of clinic to diabetic, according to the error margin standard of American diabetes academic association. And in addition to the advantages of specific, accuracy and simplicity, this method can automated analyse. So it can be expanded and used extensively.
