肾脏内分泌素及心房肽在小儿肺炎心衰发生发展中的作用

摘  要:采用放射免疫法测定45例肺炎和31例肺炎心衰婴幼儿血浆肾素、血管紧张素Ⅱ、醛固酮及心房肽浓度,并与对照组进行比较,包括两组治疗前后各项指标的动态观察。结果显示:肺炎组4项指标血浆浓度水平均高于正常对照组,肺炎心衰组均高于肺炎组,差别都有显著性。肺炎与肺炎心衰患儿治疗产后4项指标血浆浓度均呈明显下降。检测结果显示,各项指标血浆浓度随病情加重而上升,随病情好转而下降。提示:肾素、血管紧张素Ⅱ、醛固酮系统及心房肽在小儿肺炎急性期,尤其是在肺炎合并心衰期时,其血浆水下随病情而变化;并参与肺炎时和肺炎心衰时机体的病理生理变化。Plasma levels of renin(PRA) ,angiotensin Ⅱ(ATⅡ) ,aldosterone(Ald)and atrtal natriuretic peptide (ANP) were measured by radioimmunoassay in 45 children with pneumonia and 31 with pneumonia-related heart failure,and were compared with those of normal controls. During the medication against pneumonia and heart failure the 4 involved plasma levels were monitored. The results showed that all the 4 plasma levels were elevated significantly in the pneumonia group compared with the control,and were raised to the highest in children with pneumonia-related heart failure. As the children recovered from the disease through medication the plasma levels declined concomitantly. The study demonstrated that the levels of renin-angiotensin-aldosterone system (RAAS) and ANP were elevated in accordance with the progression of the disease. These findings suggested that all the 4 indexes were actively involved in the pathophysiological changes of this disease.
