摘 要:目的:探讨经过耳上弧形切口进行耳廓上部软骨松解及软骨耳轮部反卷法进行隐耳畸形矫正的手术方法。方法:隐耳畸形矫正的重点一直都是松解耳廓上部软骨,封闭创面,形成颅耳沟。但是不管是经典的术式,还是后来的新发展,都是针对颅耳沟的形成及创面的封闭的。这些术式所获得的效果都仍然存在耳廓上部的尖角问题。我们在进行此类畸形的矫正时,将耳廓上部软骨的蜷缩粘连彻底松解后,又将其与耳前皮肤的粘连做适当的剥离,并将耳软骨边缘向后反卷形成细管状,这样一般都能纠正耳廓上部的尖角畸形。然后再将弧形皮瓣向后覆盖新形成的耳轮,耳背部的创面植皮覆盖。结果:2006年3月~2008年3月我们采用这种方法为12例(15耳)患者实施手术,术后即时效果满意,随访时间最长2年,最短3个月,效果满意。结论:耳上弧形切口软骨反卷法是对传统的隐耳畸形矫正术式的重要补充,值得推广。Objective To investigate the correction of cryptotia with superior parts of ear cartilage relaxation and convolution passing arc incision. Methods In traditional idea, the correction of cryptotia includes two sides: reshaping otocranium sulcus by relaxing the superior parts of ear cartilage from the head and closing the wound, so the correction procedures, whether the classic methods or their later advancements, always were aimed at the above two keys. However, by thus operations, the ear superior part often still had some extent of angular deformation. When doing cryptotia corrective procedures, we always used superior ear arc incision, first dissected the superior parts cartilage from head and preauricular skin other then only relaxing them from the head, then convoluted the liberated cartilage rim shaping a slim roll, and then spread the arc flap to cover the new helix cartilage and the auricular back, at last closed the postauricular wound with skin graft. In general, the angular deformation could be corrected well by this method. Results From 2006, 3 to 2008, 3, we carried out corrective procedures for 12 congenital cryptotia patients (15 ears) with this method, the immediate results were satisfied. 3 months to 2 years later, the follow up results were satisfied too. Conclusion Correction of cryptotia with superior parts of ear cartilage convolution passing arc incision are important complements to the classic methods, and worthy of popularization.
