黄体酮加补佳乐用于黄体支持对IVF-ET妊娠结局的影响

摘　　要：目的：探讨黄体酮加用不同剂量戊酸雌二醇（补佳乐）用于黄体支持对IVF-ET妊娠结局的影响。方法：前瞻随机对照研究，选择392个长方案降调节后的常规IVF或ICSI周期，自采卵日起分两次肌注黄体酮80-100mg／d，自移植日起随机分别给0、2、4、6mg／d补佳乐口服作为黄体支持，计算每组早期妊娠丢失率、妊娠率、每移植种植率。结果：6mg组早期妊娠丢失率最低，妊娠率最高，但与其它组差异无显著性；种植率随补佳乐用量增加而提高，6mg组为最高达到32．7％，0mg组最低为23．5％，但仅0mg组和6mg组间差异有显著性（P〈0．05）。结论：6mg／d补佳乐用于黄体支持有可能是提高IVF或ICSI—ET周期种植率和妊娠率、降低早期妊娠丢失率的有效剂量。Objective： To evaluate the influence of pregnaney outeomes with different dosage of oestradiol valerate in addition to progesterone for luteal supplementation in IVF - ET eyeles. Methods： Prospective, randomized study was conducted to ehoose three hundred and ninety - two cycles of in vitro fertilization with a long GnRH agonist protocol; Since ooeyte retrieval injecting luteal supplementation with progesterone in oil, 80 - 100 mg/d, ira. , derided into two doses daily; Starting on the day of embryo transfer giving orally 0, 2, 4, 6 mg/ d of ocstradiol valerate; Finally calculating early pregnancy loss rate, implantation rate per embryo transfer and pregnancy rate （PR） for each group. Results： The lowest early pregnancy loss rate and higest PR were achieved in the group with 6 mg/d of oestradiol valerate, but there were no significant difference between all four groups ; Implantation rate increased with the raised dosage of oestradiol valerate, with the higest rate （32. 7%） in group of 6 nag vs. lowest rate （23. 5% ） in group of 0 rag, yet significant difference was only found between these two groups （P 〈 0. 05 ） . Conclusion： Six milligrams per day of ocstradiol valerate starting on the day of oocyte retrieval in addition to progesterone for luteal supplementation might be an effective dosage to improve implantation rate and PR, as well as to reduce early pregnancy loss rate in IVF- ET cycles with a long GnRH agonist protocol.
