血小板特异性抗体对特发性血小板减少性紫癜诊断价值的研究

摘　　要：目的检测特发性血小板减少性紫癜（ITP）及非免疫性血小板减少患者抗血小板特异性抗体与PAIg，评价其在ITP中的诊断价值。方法用流式细胞仪（FcM）检测PAIgG、PAIgA、PAIgM；用改良单克隆抗体特异性俘获血小板抗原（MAIPA）技术检测抗血小板特异性抗体GPIIb／IIIa和GpIb／IX。结果PAIg在ITP组灵敏度为80％，在非免疫性血小板减少症组为63．3%.MAIPA法检测抗GPIIb／IIIa和GPIb／IX，在ITP组中的灵敏度分别为53．3％和30％；在非免疫性血小板减少症组中除1例GPIb／IX阳性外，其余均为阴性；两组检出率比较均有差异（P〈0．05）。结论PAIg的灵敏度较高，但特异度较差；抗血小板特并性抗体灵敏度虽较低，但特异性很高，可鉴别ITP和非免疫性血小板减少。抗血小板特异性抗体的检测可成为ITP的特异性诊断指标。Objective To detect the platelet glycoprotein-specific autoantibody GPIIb/IIIa, GPIb/IX and PAIg in the blood of Idiopathic thrombocytopenic purpura patients and non-immune thrombocytopenia patients, evaluate their value in diagnosing ITP. Methods We detect the PAIgG, PAIgA, PAIgM by the means of FCMand detect GPIIb/IIIa and GPIb/IX by the technique of modified monoclonal antibody immobilization of platelet antigen. Results The positive percentage of PAIg in ITP group was 80%, which was in non-immune thrombocytopenia group was 63.3%. In ITP group the positive percentage of GPIb/IX and GPIIb/IIIa by the means of MAIPAwere respectively 30 % and 53.3%. Only one patieht in non-immune thrombocytopenia group waspositive of GPIb/IX, there was significant difference between two groups. Conclusion The sensitivity of PAIg is veryhigh but its specificity is low. The sensitivity of MAIPA is not high but the specificity is very high. Modified monoclonal antibody immobilization of platelet antigen assay （MAIPA ） can differentiate immune thrombocytopenia and non-immune thrombocytopenia. Platelet glycoprotein-specific autoantibody by MAIPA technique can be a specific index in diagnosing ITP. PAIg and MAIPA can he applicated together to strenthen the sensitivity in diagnosing ITP.
