摘  要:目的研究OEC-SC-ECM-PLGA桥接体对大鼠坐骨神经缺损的解剖学修复作用。方法建立15mm的SD大鼠右侧坐骨神经缺损模型后,用OEC、SC、ECM以及自行制备的PLGA导管构成OEC-SC-ECM-PLGA桥接体进行修复,同时设OEC-ECM-PLGA组、SC-ECM-PLGA组、ECM-PLGA组、PLGA组和自体神经移植组进行对照。术后1、3、6、9周OEC-SC-ECM-PLGA组、OEC-ECM-PLGA组和SC-ECM-PLGA组行细胞示踪检测,在术后9周各组行神经连接率、再生神经相对直径恢复率及超微结构检测。结果OEC和SC可在桥接体内至少存活6周,SC的存活率高于OEC,两者沿神经纵轴呈梭形分布,并向桥接体近心端和远心端坐骨神经内迁移至少2.5mm,OEC-SC-ECM-PLGA组的神经连接率、再生神经相对直径恢复率、有髓神经纤维密度、总神经纤维数及有髓神经纤维成熟度优于OEC-ECM-PLGA组、SC-ECM-PLGA组、ECM-PLGA组和PLGA组,还未达到自体神经移植组水平。结论OEC和SC可在缺损坐骨神经桥接体内存活,沿神经纵轴分布,并向桥接体两端坐骨神经内迁移。OEC-SC-ECM-PLGA桥接体对大鼠坐骨神经缺损具有显著的解剖学修复作用。Objective To investigate the anatomical repairing effect of self-made bridging complex of olfactory ensheathing cells, schwann cells, extracellular matrix and poly (DL-lactide-co-glycolide) (OEC-SC-ECM- PLGA) on the defect of sciatic nerve of rats. Methods The 15-ram SD rat right sciatic nerve defect model was established and repaired with OEC-SC-ECM- PLGA bridging complex, which contained OEC, SC, ECM, and self-made PLGA conduit. At the same time, OEC-ECM-PLGA group, SC-ECM-PLGA group, ECM- PLGA group, PLGA group, and nerve autograft group were set up as the controls. At 1,3, 5 and 9 weeks after surgery, the cell tracking test was performed in these groups. The nerve connective rate, relative diameter recovery rate and ultra- structure observation were performed in all groups in 9 weeks after surgery. Results OEC and SC survived at least 6 weeks in the bridging complex, and SC had higher survival rate than OEC, both cell types distributed longitudinally along the nerve and demonstrated spindle morphous, migrated at least 2.5 mm toward proximal and distal ends of bridging complex in rat sciatic nerve defect. Compared to OEC-ECM-PLGA group, SC-ECM-PLGA group, ECM-PLGA group and PLGA group, OEC-SC-ECM-PLGA group showed better results in nerve connective rate, relative diameter recovery rate, the fiber density, the total nerve fiber number and maturity of myelinated nerve fiber, but all of these were not as well as in nerve autograft group. Conclusion OEC-SC-ECM-PLGA bridging complex has significantly anatomical repairing effect on sciatic nerve defect in rats.
