摘  要:目的:报道侧前路减压钛网植骨融合内固定治疗椎管明显占位胸腰椎爆裂性骨折的临床应用效果。方法:对16例椎管明显占位胸腰椎爆裂性骨折行侧前路减压,钉板或钉棒固定加椎间钛网融合器植骨融合固定。结果:16例均获随访12~32个月。手术后胸腰椎椎间隙高度及生理曲度恢复满意,平均Cobb's角及平均椎管狭窄指数得到明显改善,神经功能恢复(Frank分级)提高1~3级。2例术中损伤胸膜并给予修补,5例术后出现腹股沟区麻木,未发生其他并发症。结论:侧前路减压钛网植骨融合内固定治疗椎管明显占位胸腰椎爆裂性骨折可获得良好临床疗效。Objective: To report the clinical effects of anterolateral decompression and fixation with titanium mesh cage on thoracolumbar burst fractures. Mothods: Sixteen patients suffering from thoracolumbar burst fractures with spinal placeholder were treated with anterolateral decompression and fixation using the plate or pedicle screw with titanium mesh cage. Results: Except of 2 patients injuried pleura during the operation, and 5 suffered inguinal zone paralysis, other patients followed up about 12-32 months showed satisfactory recovery of the height of intervertebral space and the curvature of thoracolumbar vertebrae, while the average cobb' s angle, spinal canal index and Frankle class improved remarkably. None of the patients developed severe complications. Conclusions: Anterolateral decompression and fixation with titanium mesh cage is an effective method for treating thoracolumbar burst fractures with spinal placeholder.
