摘  要:背景:以往关于壳聚糖的生物相容性研究多局限于神经细胞的体外实验,而壳聚糖在神经系统体内尤其是在脊髓局部的相容性尚不清楚。目的:观察壳聚糖在大鼠脊髓组织内的降解率及生物相容性。设计、时间及地点:随机对照动物实验,于2005-08/2006-07在武汉协和医院泌尿外科实验室完成实验。材料:健康成年SD大白鼠36只,随机分为壳聚糖组和对照组,每组18只。方法:在壳聚糖组和对照组大鼠脊髓分别植入壳聚糖膜和医用丝线。主要观察指标:于术后1,2,4周测定壳聚糖膜质量,计算降解率;观察2组大鼠植入区局部炎细胞数量变化,评估生物相容性。结果:壳聚糖可在大鼠体内逐渐降解,术后1,2,4周降解率分别为14.8%,18.9%,25.0%。壳聚糖植入局部炎细胞计数高于对照组(P<0.05),但同属异物炎症反应,并无明显的全身不良反应。结论:壳聚糖膜易于降解并与脊髓组织有良好的生物相容性,是一种可应用于脊髓损伤修复的生物材料。BACKGROUND: The past studies on biocompatibility of chitosan mainly focus on in vitro experiment, it is unclear if chitosan has good biocompatibility in vivo, especially that in local area of the spinal cord. OBJECTIVE: To study the biodegradation and biocompatibility of chitosan film in the spinal cord. DESIGN, TIME AND SETTING: The randomized controlled animal experiment was carded out in the Urology Laboratory of Wuhan Union Hospital from August 2005 to July 2006. MATERIAL: Thirty-six adult Sprague-Dawley rats were randomly divided into experimental group and control group, eighteen rats in each goup. METHODS: Chitosan film and silk suture were respectively transplanted into spinal cord of rats in the two groups. MAIN OUTCOME MEASURES: At 1, 2, 4 weeks after surgery, the weight of chitosan film was measured to calculate the rate of degradation, and biocompatibility was assessed by observing local inflammatory reaction. RESULTS: The biodegradation rate of chitosan film were 14.8%, 18.9%, 25.0% after the transplantation at 1, 2 and 4 weeks, respectively. The amount of inflammatory cells of local tissues in the experimental group were significantly higher than those in the control group (P 〈 0.05), but there were no obvious systemic reaction. CONCLUSION: Chitosan films can be easily biodegraded and has good biocompatibility in the spinal cord, which can be used in repairing spinal cord injury.
