摘  要:背景:低功率激光照射能促进骨骼肌损伤愈合的结局早已公认,但其确切作用机制仍不十分明确。目的:通过635nmGaAlAs激光照射急性骨骼肌钝挫伤大鼠,观察腓肠肌Ⅰ型胶原、Ⅲ型胶原mRNA的表达。设计、时间及地点:随机对照动物实验,于2006-07/2007-05在苏州大学体育学院中心实验室和苏州大学附属第一医院完成材料:健康雄性SD大鼠96只,体质量180~220g,随机分为自然愈合组54只和激光治疗组42只。根据取材时间进一步分为:正常对照组、损伤后即刻组及损伤后7个时间点,每组及每个时间点6只。方法:建立大鼠急性骨骼肌钝挫伤模型,激光治疗组采用美国Erchonia EML(ML2)型低功率GaALAs激光器在钝挫伤局部进行照射,自然愈合组不进行照射。主要观察指标:于损伤后即刻、伤后第1,2,3,7,14,21,28天以实时荧光定量聚合酶链反应法观察Ⅰ型、Ⅲ型胶原mRNA表达,以光镜观察组织学改变,以黄嘌呤氧化酶法测定腓肠肌超氧化歧化酶活力、硫代巴比妥酸法测定丙二醛含量结果:96只大鼠全部进入结果分析。①苏木精-伊红染色显示,激光照射组肌肉再生速度、愈合质量显著优于自然愈合组。②急性钝挫伤后,肌组织中超氧化物歧化酶活性降低,丙二醛含量增加,在损伤后第7天仍未恢复至正常水平;激光照射组上述指标变化幅度小于自然愈合组,在损伤后第7天已恢复至正常水平。③急性钝挫伤后,肌组织中Ⅰ型、Ⅲ型胶原mRNA表达增加,激光照射组升幅显著小于自然愈合组。结论:结局证明,635nmGaAlAs低功率激光能阻止胶原纤维的过度生成,抑制活性氧生成,减轻脂质过氧化损伤,促进肌肉再生。BACKGROUND: Low energy laser irradiation can promote recovery of skeletal muscle injury, but the mechanism is still uncertain. OBJECTIVE: To investigate the effects of 635 nm GaAIAs laser on the oxidative stress and expression of collagen- Ⅰ and collagen-Ⅲ mRNA in rat muscle contusion models. DESIGN, TIME AND SETTING: Randomized controlled animal experiment was performed at the Central Laboratory of School of Physical Education and Sports Science of Soochow University and First Hospital of Soochow University from July 2006 to May 2007. MATERIALS: Ninety-six healthy male SD rats weighing 180-220 g were randomly divided into natural healing group (n=54) and laser irradiation group. (n=42). According to the time of sampling, the two groups were further divided into control group, immediate after injury group, and 1, 2, 3, 7, 14, 21, 28 days after injury groups (n=6). METHODS: Rat muscle contusion models were established. The injured muscle of laser irradiation groups were irradiated by Erchonia EML (ML2) laser made in USA, and the natural healing group was not irradiated. MAIN OUTCOME MEASURES: Real time PCR technique was used to determine the expression of collagen Ⅰ and Ⅲ mRNA; optical microscope was used to observe histological changes, and superoxide dismutase (SOD) content in gastrocnemius was determined with xanthine oxidase method, and malondialdehyde (MDA) concentration was determined with thiobarbituric acid method. RESULTS: Ninety-six rats were all included in the final analysis. HE staining showed that, low energy laser promoted the regeneration after skeletal muscle injury, and accelerated the healing process. After injury, SOD activity was decreased, while the MDA content was increased, and did not restore to normal levels on day 7. Low energy laser irradiation reduced the changes of SOD and MDA, and both levels restored to normal on day 7.
