扩张型心肌病左心功能三维超声心动图10例分析

摘　　要：目的：探讨三维超声心动图（３ＤＥ）对扩张型心肌病左心功能的诊断价值。方法：应用３ＤＥ图像再建技术，对１０例扩张型心肌病（ＤＣＭ）患者、１０例正常人（ＮＳ）进行左心功能研究，并与二维超声心动图（２ＤＥ）对照。结果：３ＤＥ、２ＤＥ两种方法所测左室容量（ＬＶＶ）、射血分数（ＥＦ），在ＮＳ组二者无差别（Ｐ＞０．０５），在ＤＣＭ组，均有差别（Ｐ＜０．０１）。３ＤＥ、２ＤＥ对ＮＰ组与ＤＣＭ组的ＬＶＶ与ＥＦ测值，两组差别显著（Ｐ＜０．０１）。ＮＳ组低节段收缩率（＜１０％）占总面积的８．４８％，而ＤＣＭ组占５０．５９％（Ｐ＜０．０１）。结论：在左室形状有变化的病理心脏，３ＤＥ对心功能参数的测定具有相当好的准确性，明显优于２ＤＥ。Objective:To analyse left ventricular function with 3 dimensional echocardiograph(3 DE) in patients with dilated cardiomyopathy(DCM).Methods:The left ventricular(LV) function of 10 patients with dilated cardiomyopathy DCM and 10 normal subjects(NS) was studied with 3 DE in comparison with 2 DE.Results:In NS group,LV volume (LVV) and ejection fraction (EF) showed no significant difference( P >0.05) between 3 DE and 2 DE.However,with in the DCM group the difference was significant( P <0.01).The LVV and EF estimated by 2 DE and 3 DE between the NS and DCM groups,showed a significant difference( P <0.01).The low contraction rate region(<10%) accounted for 8.48% of the total area in NS group and 50.59% in DCM group ( P <0.01).Conclusions:The results indicate that 3 DE is more accurate in estimating LV function than 2 DE,especially in the patients with deformed LV.
