彩色M型超声对原发性高血压病人左心室舒张功能变化的评价

摘　　要：目的利用彩色M型超声评价原发性高血压病人左心室舒张功能的变化，阐明左心室舒张早期血流传播速度（Vr）与左心室几何构型改变之间的关系，探讨其临床意义。方法选择原发性高血压病人102例和健康志愿者36例，在心尖四腔心切面，应用彩色M型多普勒超声测量Vp。结果高血压各组vP均小于对照组（F=80．99，q=3．47-20．35，P〈0．05），高血压各组按正常构型、向心性重构型、向心性肥厚型和离心性肥厚型变化顺序Vp逐渐减小，差异有显著性（q=3．67-17．36，P〈0.05）。结论Vp能够准确反映高血压左心室重构过程中舒张功能受损程度，对于高血压左心室舒张功能的正确估测具有重要临床价值。Objective To assess left ventricular diastolic function in patients with essential hypertension and interpret its relationship between left ventricular early diastolic flow propagation velocity （Vp） and left ventricular geometric configuration and its clinical significance. Methods This study consisted of 102 patients and 36 healthy volunteers. Vp was measured by color Mmode echocardiography from the apical four chamber view. Results Vp in each of hypertensive groups was significantly lower than that in control group （F=80.99,q=3.47-20.35,P〈0.05）. There were significant step-down changes among normal geometric pattern, concentric remodeling, concentric hypertrophy and eccentric hypertrophy in Vp （q=3.67-17.36, P〈0.05）. Conclusion In essential hypertension, Vp reflects the degree of left ventricular diastolic dysfunction along with the progression of left ventricular configuration, which is of great value in assessment of left ventricular diastolic function in hypertension.
