超声引导下经阴道穿刺治疗未破裂型输卵管妊娠的临床分析

摘　　要：目的探讨超声引导下经阴道穿刺注射甲氨蝶呤(MTX)治疗未破裂型输卵管妊娠的疗效和临床价值。方法将46例诊断为未破裂型输卵管妊娠的患者行超声引导下经阴道穿刺注入甲氨喋呤(MTX)30mg进行治疗(穿刺治疗组),将27例未破裂型输卵管妊娠患者一次性肌肉注射甲氨喋呤50mg进行治疗(对照组)。比较两组的治愈率、血β-HCG的定量变化、附件包块的变化情况及毒副反应的发生率,对其结果进行统计学分析。结果穿刺治疗组治愈率及修正后治愈率均高于对照组,差异有统计学意义(P<0.05);穿刺治疗组血β-HCG下降至正常范围的时间比对照组短,差异有统计学意义(P<0.05);两组成功病例附件包块完全吸收时间差别无显著性(P>0.05)。结论超声引导下经阴道穿刺治疗未破裂型输卵管妊娠比单次肌注MTX能取得更好的疗效且无明显的并发症、预后良好,适用于要求保留生育功能的妇女。OBJECTIVE To observe the therapeutic effect and the clinical value of ultrasound-guided transvaginal Centesis to treat unbroken tubal pregnancy. METHODS The treatment group： 46 cases of tubal pregnancy not rupture was treated by ultrasound-guided transvaginal eentesis. The control group ： 27 cases treated by MTX single intramuscular injection, to periodic monitor blood-HCG and the mass to normal, and compare the cure rate and side effect rate of the two groups. RESULTS Tile cure rate and amended cure rate of the two groups showed markedly significant difference（P〈0.05）. Compare with control group, treatment group obviously shortened the period of blood-HCG reaching normal level（P〈0.05）. CONCLUSION Compare with single intramuscular injection, ultrasound-guided transvaginal eentesis to treat unbroken tubal pregnancy is more effective and none side effect. It is suitable to the women who want repregnancy.
