摘 要:目的评价后路钉板系统在Hangman骨折中的应用价值。方法该院脊柱外科2004年11月~2006年12月共有10例采用后路钉板系统治疗的Hangman骨折。Levine-EdwardsⅠ型骨折2例,Ⅱ型骨折5例,3例Ⅲ型骨折。所有患者入院后常规行颅环弓骨牵引。全身麻醉后,患者取俯卧位,Mayfeild头架固定,行后路开放复位,后路侧块钉板系统内固定。结果所有患者均得到6~32个月的随访(平均16个月),未发现脊髓神经损伤加重征象,无明显颈椎畸形遗留,所有病例均获得骨折愈合或植骨融合;所有患者行内固定取出术后,基本恢复头颈旋转功能。结论后路钉板系统可使Hangman骨折获得良好的复位及坚强的固定,是一种安全、可靠的内固定方案。[Objective] To study and evaluate the efficacy of lateral mass fixation in the treatment of the Hang-man's fracture. [Methods] Ten patients with Hangman's fracture treated with lateral mass fixation at our hospital from Jan. 2004 to Dec. 2006 were retrospected and analysed. According to Leving-Edwards' classification on Hang-man's fracture, two cases of type I, five type Ⅱ and three type Ⅲ were found. After skull traction, all cases were operated by posterior reduction, and fixed with lateral mass fixation. [Results] All cases performed 6 to 32 months (mean 16 months) follow-up, and rehabilitated well, The complications of injuried spine cord didn't aggraviate. They got bony union or their fractures were cured entirely. Their ranges of neck rotation were restored normal after removing their implants. [Conclusion] Lateral mass fixation is an effective and safe method to treat Hangman's fracture, it can make fractures to achieve anatomic reduction and get rigid fixation.
