摘 要:目的研究环氧合酶-2(COX-2)和血管内皮生长因子(VEGF)在增生性瘢痕(HS)中的表达及其临床意义。方法将取自患者的HS组织标本32例设为A组,普通瘢痕组织标本12例设为B组,正常皮肤组织标本8例设为C组。采用SP法检测A、B、C各组中COX-2和VEGF的表达,并以权重法分别对各组COX-2和VEGF的表达进行量化。分析COX-2与VEGF表达的相关性,以及COX-2和VEGF的表达与HS临床特征的关系。结果COX-2和VEGF在A组中呈阳性表达,并显著高于B、C组(P<0.01)。COX-2与VEGF的表达呈高度正相关性(表皮层r=0.929,P<0.01;真皮层r=0.944,P<0.01)。COX-2和VEGF在A组中的表达与患者的年龄无明显关系(P>0.05),与瘢痕的隆起高度、柔韧度、充血程度、伴随症状及临床分期有关。结论COX-2和VEGF的表达与HS形成机制有关,COX-2可能通过诱导血管生成而促进瘢痕增生。检测COX-2和VEGF的表达有助于判断瘢痕的成熟程度。Objective To study the expression of cyclooxygenase - 2 (COX - 2) and Vascular endothelial growth factor (VEGF) in hypertrophic scar and its clinical significance. Methods Tissue specimens from 32 cases of hypertrophic scar, 12 cases of normal scar and 8 cases of normal skin were evaluated as groups A, B and C respectively. Immunohistochemical techniques were used to detect the expression and distribution of COX - 2 and VEGF in these 3 groups. The expression was quantified by weighted average method, and then the correlation between COX - 2 expression and VEGF expression was investigated. Correlations between expression of COX- 2 or VEGF and clinical features of hypertrophic scar were also evaluated. Results Expression of COX- 2 protein and VEGF were predominantly positive in group A. The mean values of COX- 2 and VEGF in group A were higher than those in both group B and C ( P 〈 0,01 ). COX - 2 expression was correlated significantly with EGF expression (in epidermis r =0.929, P〈0.01, in dermis r =0.929, P〈0.01). There was significant oorrelation between COX- 2 or VEGF expression and various clinical features of hypertrophic scar including height, hardness, degree of hyperemia, simultaneous phenomenon, clinical stage of scar, but not with patient age. Conclusion The mechanism of hypertrophic scar formation is associated with the expression of COX - 2 and VEGF. COX- 2 may promote the formation of hypertrophic scar by stimulating scar angiogenesis in concert with VEGF. The expression of COX - 2 and VEGF will likely be helpful for evaluating the maturity of scar.
