腹腔镜下行肾囊肿去顶术治疗体会——附8例报告

摘　　要：目的:探讨腹腔镜经腹腔、腹膜后途径治疗肾囊肿的方法与疗效。方法:气管内全麻下对8例肾囊肿患者经腹腔或腹膜后途径,腹腔镜下找到并暴露肾囊肿后,距肾实质约3～5mm处,用剪刀沿囊肿边缘环形切除肾囊肿顶壁,边缘电灼破坏。结果:本组8例全部手术成功,无中转开放性手术,手术时间40～130分钟,平均60分钟。术中出血10～70ml,平均40ml,术后24～48小时拔除引流管,无合并出血、感染等并发症。全部患者5～7天出院,平均住院时间53天。术后平均随访时间12个月,B超检查未发现囊肿复发。结论:腹腔镜肾囊肿去顶术是一种安全有效的治疗方法,具有创伤小、出血少、住院周期短等优点,易掌握,并发症几乎不发生。适用于各种肾囊肿,可以根据囊肿的位置和大小选择手术入路。Objective： To investigate the method and effecf of retroperitoneal and via abdomen hparoscopic unroofing renal cysts. Method： 8 patients with renal cysts were underwent retroperitoneal or via abdomen laparoscopic unroofing after general anesthesia. The renal cyst was found and exposed laparoscopically. The roof of all cyst walls were resected with scissors from 3-5 mm of the edge of renal parenchyma and the edges were electrical burned. Results： All the operations of 8 cases were successful with no case converting to open surgery. Operation time were 40-130min （average time was 40min）. Drainages were extracted on 24 to 48 hours after operation, with no hemorrhage and no infection. All patients were checked out in 5-7 days （average was 5.5 days）. All patients were followed 12 months, no one was found recurrence under B-ultrasound.Conclusions： Laparoscopic unroofing renal cysts is an effective and safety method, it has the characters of little trauma, little hemorrhage,short hospitalization time and few complications. It is easy to learn and adapt to all kinds of renal cysts. The operation approach can be chose by the localization and magnitude of the cysts.
