摘  要:目的:探讨慢性阻塞性肺疾病(COPD)的气道反应性及IL-8因子在COPD发病中的作用。方法:用组胺气过吸入激发试验观察缓解期COPD、哮喘和健康人的气道反应性,同时测定气道激发前后血清IL-8水平变化。结果:COPD和哮喘患者均呈气道高反应性,且COPD组PD20FEV1与FEV1/Pre%呈显著正相关(r=0.418,P<0.01);缓解期COPD和哮喘息者血清IL-8水平均显著高于正常对照组(P<0.02,P<0.05),IL-8水平与PD20FEV1无显著相关性;气道激发后COPD和哮喘血清IL-8均较激发前下降,COPD组差异不显著(P>0.05),哮喘组差异显著(P<0.02).结论:气道高反应性可能是COPD的特征之一,IL-8在COPD和哮喘的发病过程中起重要作用,并可能是缓解期的抗炎性细胞因子。Objective:To investigate the nature of bronchial responsiveness in patients with COPD and the effect of IL-8 on the pathogenesis of COPD. Methods:The airway rpactivity in subjects with COPD is studied by a bistamine inhalation test(HIT) compared with asthmatic patients and normal individuals.and the IL-8 serum concentration was determined by ELISA be fore and after HIT. Results :Both COPD and asthmatic patinets demonstrated bronchial hyperresponsiveness(BHR),and there was a positive cor-relation between the values of PD50FEV1 and FEV1/Pre % in COPD patients (r= 0. 418,P<0. 01). Sig-nificantly increased IL-8 level was found in the patients with stable COPD and asthma compared with that of the controls (P< 0.02, P< 0. 05, respectively), but there was no correlation between the in-creased IL-8 concertration and PD20FEV1 after HIT the IL-8 content in. patients with COPD and asthma decreased but not signifaicantly with CQPD (P>0. 05);and significantly with asthma(P<0. 02). Con-clusion: BHR seems to be a characteristic of COPD and IL-8 may play an important role in the pathogen-esis of COPD and asthma,and it rnay also be an anti-inflammatory cytokine in stable COPD and asthma.
