卡托普利三联治疗老年不同类型高血压的对比研究

摘　　要：目的探讨卡托普利三联小剂量单次给药对老年不同类型高血压的疗效及安全性。方法选择老年高血压患者156例,分为A、B两组,其中A组84例为老年单纯收缩期高血压(ISH),B组72例为收缩期舒张期高血压(SDH)。均给予卡托普利25mg/d,氢氯噻嗪12.5mg/d,单硝酸异山梨酯缓释片40mg/d,晨顿服,共治疗8周。分别观察治疗前、后血压、脉压、血生化指标并评价降压效果及安全性。结果治疗8周末,A组收缩压下降非常显著(P<0.01),而对舒张压影响不大(P>0.05),B组收缩压、舒张压非常显著下降(P<0.01)。脉压差A组降低非常显著(P<0.01),与B组治疗后比较有非常显著性差异(P<0.01)。结论卡托普利三联小剂量单次给药治疗老年ISH及SDH均安全有效,而对老年ISH降压效果更显著。Objective To find out the efficacy and safety of single low- dose captopril triple therapy for senile patients with different types of essential hypertension. Methods Totally 1.56 aged patients with essential hypertension were divided into two groups, 84 aged patients with isolated systolic hypertension （ISH） in group A and 72 aged patients with systolic/diastolic hypertension （SDH） in group B. All of the patients were given a test dose of 25mg oral captopril. 12.5mg hydrochloroth- iazide and 40rag isosorbide mononitrate per day in the morning. The course of the treatment was 8 weeks. Serum lipid, pulse pressure and blood biochemistry index were observed, and the antihypertensive effect and safety were summarized. Results After 8 weeks treatment, systolic pressure descended distinctly （P〈 0.01 ）, but diastolic pressure was indistinctly （P 〉0.05） in group A. Systolic pressure and diastolic pressure were both decreased significantly （P〈 0.01） in group B. The pulse - pressure- decreasing effect of group A was very significant （ P〈 0.01 ） and the difference was very remarkable as compared with that of group B （P〈 0.01）. Conclusion It is safe and effective to use single low - dose captopril triple therapy for senile patients with isolated systolic hypertension （ISH） and systolic/diastolic hypertension（SDH） ： and the therapy has a significant curative effect on senile patients with isolated systolic hypertension （ISH）.
