X射线对膝关节盘状半月板的诊断价值

摘　　要：目的探讨膝关节盘状半月板的X射线表现及参考标准。方法回顾性分析27例经关节镜证实的盘状半月板病例和40例正常膝关节的X射线表现,并对外侧膝关节间隙宽度和腓骨头高度等相关数据进行测量和对比分析。结果外侧膝关节间隙的宽度和腓骨头高度在盘状半月板和正常膝关节的两组数据间差异有显著性意义;外侧膝关节间隙5 mm及腓骨头高度11.5 mm作为阈值具有较高的敏感性和特异性,可以为诊断和排除盘状半月板提供帮助。结论X射线平片及相关测量对诊断盘状半月板有一定的参考价值。Objective To discuss the imaging appearance of discoid menisci of the knee joint,and to evaluate the diagnostic standard of discoid menisci by plain film. Methods The radiographs of 27 patients with arthroscopicaUy proved discoid menisci and 40 knees without discoid menisci were retrospectively reviewed, and some related factors such as the distance of lateral joint space,height of fibular head were measured. Results The differences in the width of lateral knee joint space and the height of fibular head between the two groups were significant. With the cutoff value （5mm in the distance of lateral joint space and 11.5mm in the height fibular head） ,the sensitivity and specificity were high in diagnosing discoid menisci and it was conducive to the diagnosis and exclusion of discoid menisci. Conclusion Plain film and relevant measurement are helpful for discoid menisci diagnosis.
