高频通气治疗吸入性损伤

摘　　要：吸入性损伤综合治疗措施中，机械通气是不可缺少的一部分，但若呼吸机参数设置不当，将导致呼吸机相关性肺损伤。机械通气引起的肺部过度牵张能够导致炎性细胞因子聚集、内皮和上皮细胞通透性增加、肺部液体平衡失调及明显肺组织损伤。为减轻机械通气所致肺损伤，近年倡用保护性通气策略治疗肺功能不全，潮气量6～8mL／kg、吸气平台压小于或等于30cmH2O（1cmH2O=0．098kPa）、High frequency ventilation （HFV） is a kind of lung protective ventilation strategy. High-frequency jet ventilation （HFJV） can decrease the water content, relocate interstitial fluid and accelerate lymph flow in the lung of dogs with smoke inhalation injury. HFJV can effectively improve breathing mechanics and gas exchange in dogs with smoke inhalation injury. Clinical application also proves that HFV is efficient in treatment of inhalation injury.
