摘  要:目的免疫组化观察TNF-α在腰椎间盘MRI高信号区中的表达。方法对26例下腰痛患者MRI的T2像HIZ区椎间盘组织和4例脊柱侧凸椎间盘组织进行TNF-α免疫组化染色观察。结果在HIZ区表达大量的TNF-α免疫阳性细胞,HIZ区TNF-α免疫阳性细胞明显多于HIZ区周边纤维环组织和对照组。(P<0.05)。结论腰痛患者腰椎间盘MRI的HIZ区中表达大量TNF-α免疫阳性细胞,表明HIZ作为致痛椎间盘的一个特异性标志物的炎症机制基础。Objective To observe the expression of TNF-α in the high intensity zone(HIZ) onMRl contained within the lumbar discs through immunohistochemical method. Methods Twenty-six disc samples of lumbar intervertebral disc with high intensity zone (HIZ) on MRI were harvested from patients with low back pain and normal samples harvested from the patients undergoing the surgery for scoliosis (n=4). Discs were immunostained using antibodies of TNF-α. Results Higher TNF- α immunoreactive cells in HIZ on MRI were observed. The number of TNF- α immunoreactive cells in HIZ was significantly higher than in annulus fibrosus arounding HIZ(P〈0.05).Conclusions HighTNF-α expression in HIZ on MRl contained within the anulus of discs in patients with low back pain may indicated that the inflammation mechanism plays important role in the pathogenesis, and HIZ on MRI could be considered as a specific sign of painful disc.
