代谢当量及呼吸睡眠监测联合观察在中度睡眠呼吸暂停低通气综合症的疗效评估中的应用

摘　　要：目的探讨呼吸睡眠监测(PSG)及代谢当量联合观察在中度睡眠呼吸暂停低通气综合症(OSAHS)疗效评估中的应用。方法对2003年1月至2006年11月间66例中度OSAHS患者按就诊时间先后顺序,单数行县雍垂腭咽成形术(UPPP+)舌根射频消融术治疗33例,双数行舌托治疗33例。两种治疗方法分别行治疗前与治疗后12月同观察参数(AHI、最低血氧饱和度、代谢当量)行配对t检验,分别对比观察两种治疗方法间同观察参数间的变化。结果33例患者经UPPP+舌根射频消融术治疗后1年总有效率达86.7%(27/33);舌托治疗后1年总有效率达84.8%(28/33)。两种治疗方法同观察参数(AHI、最低血氧饱和度、METs)治疗前后分别行配对t检验其变化有统计学意义(P<0.05);两种治疗方法治疗后行同观察参数(AHI、最低血氧饱和度、METs)配对t检验其变化无统计学意义(P>0.05)。结论呼吸睡眠监测(PSG)和代谢当量(METs)联合观察可作为中度OSAHS患者的疗效评价参数组合之一;UPPP+舌根射频消融术与舌托治疗均为治疗中度OSAHS患者的有效方法之一,且疗效相近。Objective To detemine the value of treadmill exeeercise test in predicating patients with moderate OSAHS treated by uvulopalatopharyngoplasty and tone base reduction or oral appliance. Methods 66 patients were treated by uvulopalatopharyngoplasty and tone base reduction or oral appliance. The data including PSG（ AHI.SaO2 ） and treadmile exercice test （METs）was ananysised . Results The sueeess rat（ pencentage of patients with at least 50% redueuction in AHI ） of uvulopalatopharyngoplasty and tone base reduction was 86.7% , and 84. 8% in oral appliance . The diffference （AHI. SaO2. METs or the success teat ）between the groups of postoperation or oral appliance was nonsignificant （ P 〉 0. 05 ）. Conclusion As to moderate OSAHS, the uvulopalatopharyngoplasty and tone base reduction or oral appliance may be one of the effective thrarapy. The data ineluding PSG（ AHI,SaO2 ） and treadmile exercice test （METs）may be very useful in predicating effectiveness of treatment to OSAHS.
