溴隐亭在基础催乳素正常患者中的治疗作用

摘　　要：目的:了解基础血清催乳素(PRL)正常的排卵障碍患者中潜在性高催乳素血症(OHP)和临界潜在性高催乳素血症的发生率,观察溴隐亭(CB154)促排卵治疗的临床疗效。方法:对125例基础血清PRL正常无排卵患者,行黄体生成激素释放激素(LHRH)负荷试验,筛选出潜在性高催乳素血症患者(OHP组,34例)和临界潜在性高催乳素血症患者(临界OHP组,47例),2组患者均予溴隐亭治疗,并监测排卵情况。结果:125例患者中OHP发生率27.2%,临界OHP发生率37.6%。OHP组和临界OHP组经溴隐亭治疗显效率分别为35.3%、10.6%;有效率分别为61.8%、89.4%;无效率分别为2.9%、0。OHP组显效率高于临界OHP组(P<0.05),临界OHP组有效率高于OHP组(P<0.05),两组无效率比较,差异无统计学意义(P>0.05)。结论:在排卵障碍妇女中,LHRH负荷试验可筛选出OHP患者和临界OHP患者,溴隐亭能提高排卵率,可用于治疗OHP和临界OHP的患者,治疗OHP的效果比临界OHP好。Objective：To investigate the incidence of occult hyperprolactinemia （OHP） and borderline OHP in patients with normoprolactinemic anovulation,and to estimate the curative effect of bromocriptine.Methods：OHP and borderline OHP patients were screened out by luteinizing hormone releasing hormone （LHRH） loading test. Bromocriptine was administered and follicular development was investigated.Results：34 OHP patients and 47 borderline OHP patients were screened out from 125 cases. The incidence was 27.2% and 37.6% respectively. Significant effective rate in OHP patients and borderline OHP patients was 35.3% and 10.6% respectively. Effective rate was 61.8% and 89.4% respectively. Ineffective rate was 2.9% and 0 respectively. Significant effective rate in OHP patients was conspicuously higher than that in borderline OHP patients （P〈0.05）. Effective rate in borderline OHP patients was noticeably higher than that in OHP patients （P〈0.05）.Ineffective rate between OHP patients and borderline OHP patients had not significant difference （P〉0.05）.Conclusions：In anovulatory women,LHRH loading test is helpful to distinguish OHP patients and borderline OHP patients. The first choice treatment is bromocriptine,which can increase ovulation rate. The efficacy of bromocriptine in OHP patients was better than that in borderline OHP patients.
