新诊断2型糖尿病视黄醇结合蛋白4的变化及影响因素研究

摘　　要：目的研究新诊断2型糖尿病(T2DM)血清视黄醇结合蛋白4(RBP4)变化规律并探讨其影响因素。方法对2002年4月至10月卫生部北京医院95例新诊断2型糖尿病患者,根据体重指数(BMI)分为肥胖糖尿病组(BMI≥25)50例、非肥胖糖尿病组(BMI<25)45例,并将30名正常体重非糖尿病者设为对照组。用ELISA方法检测各组空腹血清RBP4浓度,同时测定血糖、血脂、血压、胰岛素等,用胰岛素抵抗指数(HOMA-IR)评价各组胰岛素敏感性。结果肥胖糖尿病组RBP4明显高于非肥胖糖尿病组和对照组(P<0.05),而后两组RBP4比较差异无统计学意义;单因素相关分析显示,血清RBP4与腰围(W)、HOMA-IR、BMI、三酰甘油(TG)呈正相关;多元逐步回归分析发现,腰围、HOMA-IR为影响RBP4的最显著因素。结论肥胖2型糖尿病患者血清RBP4显著升高;腰围和胰岛素抵抗是影响血清RBP4的主要因素。Objective To survey the serum level changes of retinal binding protein 4 （RBP4） in newly diagnosed type 2 diabetes, and to explore related factors of RBP4 level. Methods According to body mass index （ BMI）, 95 newly diagnosed type 2 diabetes in Beijing Hospital from April to October of 2002 were divided into obese-diabetic group（ BMI ≥ 25 ）, normal-weighted diabetic group （BMI 〈 25 ） ;30 normal-weight and non-diabetic subjects were as control group（ BMI 〈 25 ）. Serum RBP4 level was measured by ELISA. Plasma glucose,lipid profiles and insulin were also measured on fasting samples, and insulin sensitivity was assessed by HOMA-IR. Results The level of serum RBP4 in obese-diabetic group was significantly higher than that of the normal-weighted diabetic group and control group（ P 〈 0. 05 ）, and there was no difference between normal-weighted diabetic group and control group. Plasma RBP4 was correlated positively with waist circumference, HOMA-IR, BMI, and TG. Multiple regression analysis showed that waist circumference and HOMA-IR were the most significant independent determinants for serum RBP4 concentration. Conclusion Serum RBP4 level is significantly increased in obese-diabetic group. Waist circumference and HOMA-IR are the main facters which influence the serum level of RBP4.
