生存素和环氧化酶-2在T细胞淋巴瘤中表达及临床意义研究

摘　　要：目的探讨生存素(Survivin)和环氧化酶-2(cyclooxygenase-2,Cox-2)在T细胞非霍奇金淋巴瘤(T cell non-Hodgkin’s lymphoma,T-NHL)中的表达及临床意义。方法对2000年1月至2006年12月中南大学湘雅医院和中南大学湘雅二医院57例T-NHL和10例良性淋巴结病变采用免疫组化SP法检测组织中Survivin和Cox-2的表达,并与相应的临床参数进行对比分析。结果Survivin和Cox-2在T-NHL中的阳性表达率分别为78.9%和57.9%,与对照组比较差异有统计学意义(P<0.05)。Survivin的表达与Cox-2的表达呈正相关(P<0.01)。Sur-vivin和(或)Cox-2阳性组5年存活率均分别低于Survivin和(或)Cox-2阴性组,差异有统计学意义(P<0.05)。多因素分析显示,Survivin和Cox-2均是T-NHL的独立预后因素。结论Survivin和Cox-2在T-NHL中表达上调以及两者之间表达呈正相关,提示Survivin和Cox-2的表达与T-NHL的发生发展有关,且提示预后不良。Objective To investigate the expression of survivin and cyclooxygenase-2 （ Cox-2 ） in T-cell non-Hodgkin＇s lymphoma（T-NHL） and to explore its clinical significance. Methods From Janauary 2000 to December 2006, in the Xiang-ya Hospital, Central South University and the Xiang-ya Second Hospital, Central South University the expression of survivin and Cox-2 in 57 T-NHL and l0 cases of benign lymphopathy were detected by immunohistochemistry. Results The positive rate of sutvivin or Cox-2 was 78.9% and 57. 9% in T-NHL respectively（P 〈0. 05）. The expression rate of survivin was positively correlated with that of Cox-2 in tissues of T-NHL（ P 〈 0.01 ）. The patients with positive survivin expression and/or Cox-2 expression had poorer 5-year overall survival than corresponding negative patients（ P 〈 0. 05 ）. Cox regression model analysis showed that survivin and Cox-2 were both independent prognostic indexes of T-NHL. Conclusion There are higher expression for survivin and Cox-2 in the T-NHL, a positive correlation being presented between survivin and Cox-2 expression. It suggests that both survivin and Cox-2 expression in T-cell NHL tissue may be associated with the pathogenesis of T-NHL and predict poor prognosis.
