冠心病室壁运动异常对二维及三维超声心动图左室收缩功能测量的影响

摘　　要：应用多平面经食管三维超声心动图(3DE)测量了6例冠心病室壁运动正常(A组)及24例室壁运动异常患者(B组)的左室舒张末期容量(EDV)、收缩末期容量(ESV)、心搏量(SV)及射血分数(EF),并与二维超声心动图(2DE)及左室造影(LVA)的测值进行了对比,结果显示;①在A组患者中,2DE和3DE测量的EDV、ESV、SV及EF均与LVA相应测值呈高度相关;在B组患者中,3DE所测各项指标与LVA结果仍呈高度相关,而2DE与LVA相应测值呈中度相关;②2DE、3DE及LVA三种方法所测B组患者的ESV较A组患者明显增大(P<0.01),而EF则显著下降(P<0.01)。结论:左室节段性室壁运动异常是二维超声心动图心功能测值的主要误差因素,而对三维超声心动图测值无显著影响。Using multiplane transesophageal three-dimensional echocardiography(SDE) ,EDV,ESV,SV and EF were measured within 24 h in 6 cases of coronary disease without regional wall motion abnormalities (RWMA) (group A) and 24 patients with RWMA (group B),and were compared with those by means of transthoracic two-dimensional echocardiography(2DE) and left ventricular angiography. The results showed that:In group A,there were higt correlations of EDV,ESV,SV and EF measured by 2DE,3DE and LVA;① In group B,the correlations of EDV,ESV,SV and EF measured by 2DE and LVA were only moderate,but there were still high correlations of these parametrs measured by 3DE and LVA;② ESV was increased and EF decreased significantly in group B compared to those in group A (P<0. 01). in conclusion,3DE provides a highly feasible approach to the measurement of the left ventricular volume and EF.and is superior to 2DE in quantitating left ventricular systolic function in coronary heart disease patients with RWMA.
