芳香化酶P450诊断不孕症中子宫内膜异位症的初步研究

摘　　要：目的检测不孕中子宫内膜异位症(内异症)细胞色素芳香化酶P450(P450)的表达,探讨P450在内异症的发病作用及用于不孕症筛查的可行性。方法应用免疫组化方法检测不孕症中内异症和非内异症组在位内膜芳香化酶P450的表达。结果不孕症中内异症组和非内异症组在位内膜P450的表达差异有统计学意义。P450在内异症的增殖期和分泌期的表达差异无统计学意义,并与AFS分期无关。P450用于诊断不孕症中内异症的敏感度为90%,特异度为86%,阳性预测值为86.5%,阴性预测值为89.6%。结论P450子宫内膜异位症的发生、发展中起着重要作用,P450可用于不孕症中子宫内膜异位症的筛查。Objective To study the role of aromatase cytochrom P450 in the pathogenesis of endometriosis and the practicability of it serving as a diagnostic marker for endometriosis in infertility. Methods The expression of aromatase cytochrom P450 in eutopic endometrium of endometriosis and non - endometriosis of infertility was determined by immunohistochemical assay. Results There was a significant difference in the expression of aromatase cytochrom P450 between eutopic endometrium of endometriosis and non - endometriosis. There was no difference between proliferative and secretory endometrium of endometriosis. No correlation was observed between the expression of aromatase cytochrome P450 and AFS stage of endometriosis. As a diagnostic test for endometriosis by measuring aromatase cytnchrome P450 in eutopie endometrium of endometriosis in infertility , the sensitivity, specificity, positive predictive value and negative predictive value were 90% ,86% , 86.5% and 89： 6%. Conclusion The expression of aromatase cytochrom P450 in endometrium may play an important role in pathogenesis of endometriosis and can be used as a diagnostic test for endometriosis in infertility.
