摘　　要：目的探讨经尿道选择性绿激光前列腺汽化术（PVP）治疗高龄高危良性前列腺增生（BPH）的临床疗效及安全性。方法采用PVP治疗高龄高危BPH患者30例，观察平均手术时间、术中出血量、术后留置导尿管时间、手术并发症，记录并计算手术前后国际前列腺症状评分（IPSS）、生活质量评分（QOL）、剩余尿量及尿流率改变等指标的差异。结果30例均安全渡过围手术期，手术时间30～90min，术中出血量少，术后留置尿管时间24～48h。术前平均最大尿流率8.2ml／s，术后2周及4周平均尿流率分别是19.8ml／s及19．2ml／s术前IPSS和QOL评分分别为23．9分和42分，术后2周及4周分别下降至8.9、8.5分和1.4、1.4分。剩余尿量由术前平均50．8ml下降至术后10．5ml术后近期疗效满意，无严重并发症。结论PVP是治疗高龄高危BPH的一种安全有效的微创手术，手术操作简单，时间短，出血少，术后恢复快。Objective To explore the efficacy and safety of greenlight photoselective vaporization of prostate （PVP） in high - risk olderpatientswith benign prostatic hyperplasia （BPH）. Methods A total of 30 high - risk older patients with BPH underwent PVP The operative tine, blood loss, indwelling catheterization and operative comp lications were observed The variables such as IPSS, QOL, uroflowm etry, postvoid residual urine volume （RUV） were recorded and calculated pre - and postoperatively Results All the 30 patients safely got through perioperative period The operative tine was 30 - 90 rain, postoperative indwelling catheterization was 24 - 48 h Themean MFR increased from 8 2ml/s to 19. 8 ml/s（2 weeks later）, 19. 2 ml/s（4 weeks later）. Themean IPSS and QOL values decreased from 23. 9 and 4 2 to 8.9 and 1. 4（2 weeks later）, 8 5 and 1. 4（4 weeks later） respectively Themean RUV decreased from 50. 8 ml to 10. 5 ml Therewere few complieations and great patients satisfaction with the shorttern postoperative results Conclusion The PVP is a safe, efficacious andminmally invasive procedure for olderpatientswith high - risk BPH. It is easier to manipulatewith advantages of shorter operative tine, less blood loss, better to lerance and rapid recovery
