心力衰竭心阳虚证大鼠与心钠素的相关性研究

摘　　要：目的:探讨心力衰竭(chronic heart failure,CHF)心阳虚证大鼠心钠素(atrial natriuretic polypeptide,ANP)与心阳气(xin yang qi)之间的相关性及临床意义。方法:以腹腔注射盐酸阿霉素的方法建立大鼠心力衰竭(心阳气虚)模型,另设正常对照组;用放射免疫法测定血浆ANP含量,经颈动脉测量左室收缩压(left ventricular systolic pressure,LVSP)、左室舒张末压(left ventricular end-diastolic pressure,LVEDP)、左室压力最大上升及下降速度(left ventricular developed pressure,±LVdp/dt)心功能指标,测定心肌(左室)匀浆上清液丙二醛(malondialdehyde,MDA)、过氧化物歧化酶(superoxide dismutase,SOD)含量;病理学切片检测,HE染色,显微镜观察;放射免疫法测定心肌(心房)匀浆上清液ANP含量。结果:CHF组大鼠血浆ANP含量显著升高(P<0.01),LVSP、±LVdP/dtmax下降(P<0.01),LVEDP升高(P<0.01),心肌MDA含量升高(P<0.01),心肌SOD活性含量降低(P<0.01),心肌ANP含量升高(P<0.01);CHF时血浆ANP含量与心功能(+LVdp/dtmax)(心阳气)呈负相关:r=-0.775(P<0.05)。结论:ANP有可能成为评估心力衰竭心阳虚证心阳气水平的指标之一。Objective： To investigate the relationship between atrial natriuretie polypeptide （ANP） and xin yang qi（positive vital energy of heart） of rat with chronic heart failure. Methods： Chronic heart failure（xin yang qi xu） model of rats by the method of intraperitoneal injection of doxorubicin hydrochloride （ADR）, and control group was set up.Plasma ANP was determined by radioimmunoassay （RIA）; left ventricular systolic blood pressure （LVSP）, left ventricular end-diastolic pressure （LVEDP）, and the maximum increased or declined rate of left ventricular pressure （±LVdp/ dt） were determined the carotid artery. Cardiac （LV） homogenate supernatant malondialdehyde （MDA） and superoxide dismutase （SOD） content were determined.Pathologic examination was done by HE staining, and microscopy; And homogenate supernatant （Atrium） ANP was assayed by RIA. Results： Plasma ANP in CHF rats were significantly higher （P 〈 0.01）, LVSP, ± LVdP/dtmax declined （P 〈 0.01）, LVEDP increased （P 〈 0.01）, myocardial MDA increased （P 〈 0.01）, myocardial SOD activity decreased （P 〈 0.01）, and myocardial ANP was increased（P 〈 0.01）; Plasma ANP content and cardiac function （＋LVdp/dtmax）（xin yang qi） were negatively correlated in CHF rats： r = - 0.775 （P 〈 0.05）. Conclusion： ANP can be used as a possible indicator to assess the level of xin yang qi in chronic heart failure.
