烧伤患者肌松药用量的临床研究

摘　　要：目的探讨烧伤患者多次削痂植皮麻醉中非去极化肌松药阿曲库铵的用量变化。方法选择55例烧伤面积〉30％的深Ⅱ-Ⅲ度烧伤患者：分为第1、2、3次削痂植皮组，与同期50例骨科创伤患者进行对照，用国产苯磺酸阿曲库铵气管插管和术中肌松维持，记录麻醉过程中阿曲库铵的用量。结果烧伤患者第1次削痂植皮与对照组的肌松药用量无显著差异；第2次及第3次前痂植皮与第1次及对照组的用药量变化有显著统计学差异（P〈0．01）。结论。烧伤患者多次削痂植皮手术麻醉时阿曲库铵的用量明显增大．机体对非去极化肌松剂耐药。Objective To investigate the clinical dosage of non-depolarizing muscle relaxants atravurium applied to bum patients during crust excision and skin grafting. Methods A total of 55 patients with second and third degree burn （ area 〉 30% ） were enwiled and divided into one-time excision group, two-time excision group and three-time excision group. Skin grafting was performed onto all the patients. At last, comparisons with control group （50 traumatic osteopathic patients） were done in the same period. General anesthesia with atravurium was performed via tracheal intubation, atravurium＇ dosage was recorded. Results There was not significant difference in the dosage of the muscle relaxant between the one-time excision group and the control group, but the significant difference existed between the two-time excision group or the three-time excision group and the one-time excision group or the control group （P 〈 0.01 ）. Conclusion The atravurium dosage is increased apparently duririg anesthesia for multiple crust excisions （ plus grafting） in the patients with large area burn because of the drug resistance against the non-depolarized muscle relaxant.
