直接凝血酶抑制物的测定及其临床意义

摘　　要：目的采用稀释血浆凝血酶时间法(稀释纠正法)检测血浆中特异性凝血酶抑制物,为抗血栓形成治疗提供有效的实验室监测试验。方法延长的凝血酶时间(TT)用正常人混合血浆作1:4稀释后作TT测定。结果稀释纠正法测的直接凝血酶抑制物(direct thrombin inhibitors,DTIs)浓度与TT有良好的相关性(r=0.9569),可测最低DTIs浓度为8.0×10-3μg/ml,测得凝血酶抑制药物阿加曲班治疗患者DTIs血浆浓度为118.2±20.78×10-3μg/ml,不明原因轻度出血异常、急性白血病患者也存在不同程度的DTIs。结论稀释纠正法TT测定对DTIs有较高的特异性和敏感性,是凝血酶抑制物类药物治疗血栓性疾病有价值的实验室监测指标。Objective To detect specific thrombin inhibitors with a plasma diluted thrombin time and find an effective method for laboratory monitor of anticoagulant and thrombolytie therapy. Methods Patient samples were 1：4 dilutions with normal plasma and the diluted thrombin time was examined by ACL-9000. Results The direct thrombin inhibitors （DTIs）detected by diluted thrombin time were associate with the thrombin time（r =0. 9569）,the lowest density of DTIs was 8.0 ×10^-3μg/ml,the DTIs density of patients spiked with argatroban was （118.2 ± 20.78 ）10^-3μg/ml, the DTIs were also detected in patients with un- known bleeding and acute leukemia. Conclusion Detecting DTIs with a plasma diluted thrombin time is with high specificity and sensitivity,and it is a valuable index for laboratory monitor of anticoagulant and antithrombotie treatment.
