染色体核型异常对MDS的转化及预后的影响

摘　　要：目的分析染色体核型异常对MDS的转化及预后的影响。方法通过MDS患者的染色体核型进行分析,比较不同亚型各组患者染色体核型异常的比例、染色体异常类型分布及不同核型各组患者白血病转化率、死亡率和生存时间。结果收集50例原发性MDS的患者资料。其中具有染色体核型异常者23例,占46%。所涉及的核型有5q-/-5、-7/7q-、+8、-20、+11、12p+、-Y、i(17q)、平衡易位t(8:21)、t(3:5),并有复杂异常。发生频率以+8、-7/7q-较高。IPSS评分高危组染色体异常的发生率明显高于IPSS评分低危组(P<0.05)。随访的50例转白率为32%,其中核型正常的患者转白率为18.5%;核型异常患者转白率为47.8%。核型异常者转白率明显高于核型正常者(P<0.05)。结论染色体核型异常是影响MDS患者病情进展及预后的重要指标,不同的核型异常与MDS的转归有直接的关系,对MDS预后的判断有重要价值。Objective To analyze effects of abnormal karyotypes on transformation of myelodysplastic syndromes.Methods The rate of abnormal karyotypes,the distribution of abnormal karyotypes types and the leukemia transformation rate,death rate and survival time were compared in different groups by analysis of abnormal karyotypes of myelodysplastic syndromes patients.Results 23 cases（46%） had abnormal karyotypes,The abnormal karyotypes were 5q-/-5,-7/7q-,＋8,-20,＋11,12p＋,-Y,I（17q）,t（8：21） and t（3：5） and complex abnormality.The frequencies of ＋8、-7/7q-was highest.The occurrence rate of abnormal chromosome was higher in high risk group of IPSS grade than that of in low risk group（P〈0.05）.There were 32% patient of 50 cases turn to leukemia,the abnormal chromosome patients rate of turning to leukemia was 47.8%,the normal chromosome patients rate of turning to leukemia was 18.5%,the abnormal chromosome patients rate of turning to leukemia was higher than that of the normal（P〈0.05）.Conclusion The chromosome karyotypes abnormal is important to evaluate on transformation of myelodysplastic syndromes.Different types of abnormal karyotypes is related to transformation of myelodysplastic syndromes.
