摘 要:目的:探讨多囊卵巢综合征(polycystic ovary syndrome,PCOS)患者糖、脂代谢特征及身体脂肪分布特点。方法:选取多囊卵巢综合征患者31例,肥胖组(Ob-PCOS)17例,非肥胖组(Nob-PCOS)14例及年龄相匹配正常生育年龄妇女64例为对照组,进行生殖内分泌、糖、脂代谢指标检测,并采用双能X线吸收仪(DXA)测量全身和局部身体成分,计算"男性"与"女性"区域脂肪百分比的比值(A/G)。结果:肥胖组PCOS患者睾酮(T)、空腹胰岛素(Ins)、胰岛素敏感指数(ISI)、甘油三酯(TG)高于非肥胖PCOS组、对照组(P<0.01),差异有统计学意义,非肥胖PCOS组ISI、血糖高于对照组(P<0.01),差异有统计学意义。对照组HDL高于PCOS组(P<0.01),差异有统计学意义。PCOS组男性脂肪量、A/G高于对照组,差异有统计学意义(P<0.01),PCOS组间亦有差异。男性脂肪量与ISI成负相关(r分别-0.672,P<0.01)。结论:①PCOS组糖、脂代谢存在异常,存在胰岛素抵抗,肥胖组更加明显;②PCOS组脂肪分布呈男性脂肪分布模式,男性脂肪量与胰岛素抵抗有相关性。Objective: To investigate the characteristics of the glucose and lipid metabolism and body fat distribution in women with polycystic ovary syndrome (PCOS) . Methods: 31 infertile women with PCOS (14 in obese group and 17 in the non -obese) and 64 controls in reproductive age were recruited with matched age. Peripheral blood were detected for endocrinological and biochemical measure- ments. Body composition were determined by dual - energy X - ray ( DXA), Parameters provided by the software were as following : total body and local regions body composition and A/G were calculated. Results: ①The levels of fast insulin, ISI, TG and T of obese women with PCOS were higher than those of the Nob - obese group (P 〈 0. 01 ) and the control (P 〈 0. 01 ). The levels of ISI, fast glucose in the Nob - PCOS group were higher than the control group (P 〈 0. 0l ) . The level of HDL in the control group was higher than the PCOS group (P 〈 0. 01) . ③The ratio of android body fat distribution (BFD) to feminine BFD had a negative correlation with insulin sensitivity index (ISI) (r = -0. 672, respectively, P 〈 O. 01 ) . Conclusion: There are apparently abnormal in obese women with PC OS in term of glucose and lipid metabolism and insulin resistance. PCOS patient also shows android body fat distribution, and android BFD relate to the degree of insulin resistance.
