摘  要:目的应用胃液稳定微泡实验(stable microbubble test,SMT)早期诊断早产儿肺透明膜病(Hyaline membrane disease,HMD)。方法选取39例肺透明膜病患儿作为试验组,另选取羊水吸入综合征23例和湿肺17例作为对照组。分别抽取胃液做SMT,比较SMT在各组中的阳性率,并计算SMT在早期诊断早产儿HMD的敏感度、特异度、阳性预示值、阴性预示值。结果SMT在HMD中的阳性率为92.3%(36/39),明显高于对照组,差异有极显著意义(P<0.01)。SMT早期诊断HMD的灵敏度、特异度、阳性预示值和阴性预示值分别为:92.3%(36/39)8、7.5%(35/40)、87.8%(36/41)、92.1%(35/38)。结论SMT操用简便、结果快速、成本低,敏感度高,特异性好,能早期诊断早产儿肺透明膜病,对是否选用肺表面活性物质进行治疗具有指导意义,值得推广应用。Objective Tiffs study aimed to adopt Stable Microbubble Test (SMT) in early diagnosis of Hyaline Membrane Disease (HMD) in premature infants. Methods 39 cases of newborn infants with HMD were selected as a test group while 23 cases with Amuiotic Fluid Aspiration Syndrome and 17 cases with wet lung were chosen as a group for comparison. Gastric juice was taken for SMT from the newborn infants after admission for positive rates comparison between the two groups and the sensitivity, specificity, positive and negative predictive values of stable microbubbles in early diagnosis of HMD were counted. Results The positive rate of HMD is 92.3 % (36/39) much higher than that of the group for comparison, which had prominent significance ( P 〈 0.01 ). The sensitivity, specificity, positive and negative predictive values of stable microbubbles in early diagnosis of HMD were 92.3% (36/39), 87.5% (35/40) ,87.8% (36/41) ,92.1%(35/38)respectively. Conclusion SMT on gastric juice is a simple,rapid procedure with low cost,high sensitivity and good specificity to predict and make an early diagnosis of neonatal FIMD. It is helpful for deciding whether to adopt exogenous surfactant replacement during treatment and its application deserves popularization.
