腹膜后腹腔镜治疗单纯性肾囊肿(附45例报告)

摘　　要：目的讨论腹膜后腹腔镜手术治疗单纯性肾囊肿的方法及疗效。方法回顾分析45例腹膜后腹腔镜手术治疗单纯性肾囊肿的临床资料。结果手术时间50～145min,平均70min。出血量10～75mL,平均40mL。住院日5～7d,平均5.5d。中转开放手术1例,术中腹膜破裂4例,皮下气肿2例,术后漏尿2例。无输血病例,未发生严重并发症。随访3～12个月,2例有不同程度残留和复发。结论腹膜后腹腔镜手术治疗单纯性肾囊肿创伤小,康复快,疗效确切。Objective To investigate the technique of retro - peritioneal laparoscopic operation in treatment of renal simple cysts. Methods The clinical data of 45 cases of simple cysts of kidney treated by retro - peritioneal laparoscopic unroofing were analyzed retrospectively.Results The operation time was 50- 145 rain（mean,70 min） .The intraoperative blood loss was 10- 75 mL（mean,40 mL）.The postoperative hospital stay was 5- 7 days（mean,5.5 days）and the success rate of operation was 100%. 1 case was converted from the laparoscopic procedure to open surgery. Peritioneal rhexis was observed in 4 cases, pneumoderma was found in 2. leakage of urine was found in 1 case. Blood transfusion was never required. No severe complication occured. 2 case had different degree incomplete cavity and recurrence after 3 - 12 month postop re - x. Conclusion Retro - peritoneoscopic unroofing is a safe, effective, and minimally invasive treatment for simple cyst of kidney.
