配合小剂量不同疗程罗红霉素对下呼吸道绿脓杆菌感染的作用

摘　　要：目的:研究配合小剂量不同疗程罗红霉素对下呼吸道绿脓杆菌感染的作用。方法:将90例下呼吸道绿脓杆菌感染患者随机分为三组,对照组(n=30)根据药敏结果选用2种敏感抗生素联合抗绿脓杆菌,疗程3～4周,并治疗基础疾病;治疗B组(n=30)在对照组的基础上加用罗红霉素胶囊0.15g,2次/d,疗程1个月;治疗A组(n=30)在对照组的基础上加用罗红霉素胶囊0.15g,2次/d,疗程6个月。观察三组治疗前后临床疗效和细菌清除率。结果:临床总有效率对照组60.0%(18/30),治疗B组83.3%(25/30),治疗组A组93.3%(28/30),细菌清除率对照组16.7%(5/30),治疗B组40.0%(12/30),治疗A组66.7%(20/30),治疗B组临床总有效率和细菌清除率与对照组相比有显著差异(P<0.05);治疗A组临床总有效率和细菌清除率与对照组相比有极显著差异(P<0.01)。结论:配合小剂量罗红霉素长疗程治疗下呼吸道绿脓杆菌感染疗效显著,且有利于抗生素对绿脓杆菌的清除。Objective： To investigate the effect of small dosage of roxithromycin on lower respiratory tract infection of PSeudomonas acruginosa. Methods： Ninety patients who infected pseudomonas aeruginosa in lower respiratory tract were divided into 3 groups randomly： therapy A group （n=30）, therapy B group （n=30） and control group （n=30）. According to the result of pharmaceuticals sensitive ,we used a combination of two sensitive antibiotics for 3 -4 weeks, and treated the basis diseases in 90 patients.At the same time we used roxithromycin（0.15g,twice a day）for 6 months in therapy A group and roxithromycin（0.15g,twice a day）for 1 months in therapy B group. Clinical effect and bacteria clearance before and after therapy were observed. Results： There were significant differences of clinical effect （P 〈0.01）and bacteria clearance （P 〈0.01）between therapy A group and control group. There were significant differences of clinical effect（P 〈0.05）and bacteria elearance（P 〈0.05）between therapy B group and control group. The clinical utility rates were 93.3% （28/30）、 83.3%（25/30） and 60.0%（18/30） among therapy A group,therapy B group and control group, respectively.The bacteria clearance were 66.7% （20/30）、40.0%（12/30） and 16.7%（5/30） among therapy A group,therapy B group and control group,respectively. Conclusion： The effect of the long-term of therapy of small dosage roxithromyc .on lower respiratory tract infection of pseudomonas aeruginosa is remarkable.And it is advantageous for clearance ofpseudomonas aeruginosa in combination with antibiotic.
