开郁清热法对应用降糖西药治疗的肥胖2型糖尿病患者用药剂量的影响

摘　　要：目的：观察开郁清热法对应用降糖西药治疗的肥胖2型糖尿病患者用药的影响。方法：258例肥胖2型糖尿病患者采用回顾性病例调查统计方法，用SPSS12．0统计软件包进行数据分析。对治疗前后具体测定值及其变化进行分析评价。结果：经开郁清热法治疗后，258例患者中88例患者降糖西药减少用量，139例治疗方案不变，13例增加用药，18例改变用药。治疗方案不变患者治疗后空腹血糖、餐后2h血糖均较治疗前降低，有显著性差异（P〈O．01）；减少用量的患者中≤3个月的治疗前后空腹血糖、餐后2h血糖比较。无显著性差异（P〉0．05），4～6个月及〉6个月的患者治疗后空腹血糖、餐后2h血糖均较治疗前降低，有显著性差异（P〈0.01）。结论：开郁清热法可在原降糖西药用量不变的情况下有效降低空腹血糖和餐后2h血糖；可在减少原降糖西药用量之后仍能维持西药减量之前的血糖水平，甚至进一步使血糖降低，对胰岛素用量的减少作用更明显。Objective：To observe the impacts of Kai Yu Qing Re therapy （for removing stagnation and reducing heat）on dose of western hypoglycemic drugs in obese patients with type 2 diabetes mellitus. Methods ：The retrospective investigative statistical method and SPSS12.0 statistic software package were applied to analyze the data and assess the concrete determined values and their changes before and after treatment for 258 obese patients with type 2 diabetes mellitus. Resuits：After the treatment with Kai Yu Qing Re therapy,of 258 cases,the dose of hypoglycemic drug was reduced in 88 cases,no change in 139 cases,increased in 13 cases and the medication was altered in 18 cases. With the same treatment plan,The fasting plasma glucose （FPG）and 2 h postprandial glucose （2 hPG）after treatment were reduced compared with those before treatment,indicating significant difference （P〈0.01）. In the cases with dose reducing for ≤ 3 months,no sig- nificant difference presented in FPG and 2 hPG before and after treatment （P〉O.05） ;for 4-6 months and 〉6 months,the significant difference presented in comparison（P〈0.01）. Conclusion：Kai Yu Qing Re therapy can reduce effectively FPG and 2 hPG in the condition of same dose administration of western hypoglycemic drugs, and can still maintain the original PG in the condition of reduced dose administration of original western drugs;moreover, the therapy can reduce PG and apparently decrease the dose of insulin.
