维吾尔族与汉族非淋球菌性泌尿生殖系感染患者病原构成分析及耐药率比较

摘  要:目的:探讨性病发展动态,了解病原构成及耐药率的变化,为临床合理用药、提高诊治水平提供依据。方法:应用病原培养和抗原检测技术对415例(维吾尔族197例,汉族217例)非淋球菌性泌尿生殖系感染患者进行病原构成分析,并对支原体阳性标本进行12种抗生素耐药实验。结果:维吾尔族患者单一因子衣原体(CT)感染率为2.03%,解脲支原体(UU)感染率为56.85%,双因子CT+UU混合感染率为1.52%。汉族患者单一因子CT感染率0.46%,UU感染率为47.92%,双因子CT+UU混合感染率为0.92%,两民族间差异无统计学意义(P>0.05)。维吾尔族患者UU感染耐药率由高到低依次为环丙沙星、司帕沙星、交沙霉素、罗红霉素、美满霉素、四环素、强力霉素、左璇氧氟、氧氟沙星、克拉霉素、红霉素、阿奇霉素。汉族UU感染耐药率由高到低依次为环丙沙星、左璇氧氟、司帕沙星、交沙霉素、罗红霉素、强力霉素、美满霉素、四环素、氧氟沙星、红霉素、阿奇霉素、克拉霉素,两民族间耐药率除环丙沙星、氧氟沙星外差异有统计学意义(P<0.05)。结论:两个民族患者均主要为单一因子UU感染,在病原构成上无明显差异,但在感染耐药率上除环丙沙星、氧氟沙星外均有明显差异。Objective: To understand pathogenic composition and change in veneral msease, ana track the tendency of drug resistence ratio, for more effective and reasonable clinic diagnosis and treatment. Methods: Applying pathogenic culture and antigen inspection techniques to analysis pathogenic composition in 415 first-visit non-gonococcal urethritis patients (Uyghur 198, Han 217) in out-patient clinic chlamydia, and using 12 kinds of antibiotics to analyze drug resistence ratio for the mycoplasma positive samples. Results: Infection ratio were respectively 2.03G for single chfamytia traehomatis (CT), 56.85% for ureaplasma urealyticnm (UU), and 1.52G for double pathogen (CT+UU) among Uyghur patients, while the infection ratio were respectively 0.46% for single CT, 47.92% for UU, and 0.92% for double pathogen (CT+UU) among Han patients. P value was 0. 435. The drug resistence ratio from high to low were ciprofloxacin, sparfloxacin, josamycin, roxithromycin, minocycline, tetracycline, deoxycycline, L-ofloxacin, ofloxacin, clarithromycin, erythromycin, azithromycin in Uyghur patients respectively, White in Han patients, the ratio were ciprofloxacin, L-Ofloxacin, sparfioxacin, josamycin, roxithromycin, deoxycy- cline, minocycline, tetracycline, ofloxacin, erythromycin, azithromycin, clarithromycin respectively. P value was 0. 0162. Conclusion: UU is main single pathogen for Uyghur and Han urethritis patients. There is no pathogenic composition difference between the two groups (P 〉0.05), but it is obvious difference on drug resistence ratio in two groups (P〈0.05).
