大剂量阿米替林、苯巴比妥联合中毒1例分析

摘　　要：超致死量阿米替林和巴比妥联合中毒临床上少有报告。本例患者在中毒现埸立即进行基础生命支持及高级生命支持,回院后在呼吸及循环支持下,展开洗胃等毒物清除措施,经纠正顽固性心律失常和顽固性低血压,碳酸氢钠碱化血液及纠正酸中毒,持续血液灌洗(CRRT),纳络硐促醒等一系列急救监护治疗后,患者第3天恢复自主呼吸,第6天苏醒,生命体征稳定,第11天血清阿米替林浓度<200ng/L,余检验指标正常或基本恢复正常,好转出院。Combined toxicosis of super-lethal dose of amitriptyline with phenobarbital tables was reported clinically few. The poisoning patient in this case was immediately rescued at the scene by basic and advanced life support, removed toxins such as gastric lavage with the aid of breathing and cycle back to the hospital. After a series of emergency monitoring and care as correcting refractory arrhythmia and refractory hypotension, blood alkalosis and acidosis correction by sodium bicarbonate, continuous blood lavage by CRRT, and wake promotion by naloxone, the patient resumed spontaneous breathing on the 3rd day, awaked with stable vital signs the 6th day, and on the 1 lth day amitriptyline serum concentration 〈200ng / L, with the remaining test indicators of normal or almost normal, and discharged.
