顺铂腹腔化疗应用及疗效观察

摘　　要：通过药代动力学研究，观察卵巢癌及胃癌术后应用顺铂腹腔化疗的疗效及其副作用。结果表明，腹水中顺铂最大浓度为周围血中的１８．７５倍，顺铂在腹水和血中的清除半衰期分别为３９．９小时和７０．６小时，腹膜对顺铂的清除率为６．１ｍｌ／ｍｉｎ，机体对顺铂的总清除率为１５．３ｍｌ／ｍｉｎ。尿２４小时顺铂排泄量为用药量的１３．５３％，而门静脉血中顺铂浓度高于周围静脉血２．５倍。提示：顺铂能保持腹腔高药物浓度，提高腹腔化疗疗效，降低全身药物毒性反应。With the intention of observing the effect of peritoneal chemotherapy with pharmaceutical kinetics the peritoneal chemotherapy with cisplatin was performed for ovarian cancer and gastric cancer patients undergoing curative resection.The results showed that cisplatin concentrations in peritoneal cavity was 18.75 times higher than those in peritoneal blood.The half life of cisplatin in peritoneal cavity and in peripharal blood were 39.9 and 70.6 hours respectively.The clearance of the peritonium to cisplatin was 6.1ml/min.The total clearance of patients to cisplatin was 15.3ml/min.The excretion of cisplatin in urine accounted for 13.53%.The concentration of cisplatin in the blood of portal vein was 2.5 times higher than those in peritoneal blood.The results suggest that cisplatin could keep higher concentration in peritoneal cavity.The peritoneal chemotherapy has effective results and decrease the toxic reaction of the chemotherapeutic agents.
