双羟基黄酮醇对去势山羊心肌缺血后冠脉内皮功能不全的阻抑作用

摘  要:目的研究双羟基黄酮醇(3′,4′-dihydroxyflavonol,DiOHF)对去势山羊心肌缺血后冠脉内皮功能的作用。方法成年雌性山羊去势处理后通过阻断冠状动脉左前降支1 h和复通灌注7 d,并根据临再灌注前静脉注射药物的不同分为两组(n=8):赋形剂(vehicle,VEH)和DiOHF(2 mg/kg),观察缺血区冠状动脉对不同浓度硝普钠(Sodium Nitroprusside,SNP)、乙酰胆碱(Acetylcholine,Ach)及肾上腺素的反应和DiOHF对区域冠状动脉血流量(Regional myocardial blood flow,RMBF)的影响。结果在VEH组,再灌注1 d后对Ach浓度依赖的血管舒张效应明显减弱,再灌注7 d后仍无好转;对SNP的浓度依赖性反应减弱,但在再灌注7 d后基本恢复。而在DiOHF组,Ach引起的浓度依赖性反应在再灌注1 d后部分保留,再灌注7 d后完全恢复;对SNP的浓度依赖性反应在再灌注过程中基本维持。DiOHF还显著提高了再灌注1 d和7 d后缺血危险区域的RMBF。结论再灌注前一次性注射DiOHF可显著改善心肌缺血再灌注损伤后的冠脉内皮功能不全,提高缺血危险区域心肌组织水平的血流灌注。Objective To study the effect of 3', 4'-dihydroxyflavonol (DiOHF) on coronary endothelial function during myocardial ischemia and reperfusion injury in castrated goats. Methods Female goats were castrated before receiving 1 h of ischemia by occluding left anterior descending coronary artery following one week of continuous reperfusion. Animals were randomly divided into DiOHF (5 mg/kg) group and vehicle (VEH) group according to different regimens of bolus injection upon reperfusion. Coronary responses in ischemia area to Sodium Nitroprusside (SNP), Acetylcholine (Ach) and phenylephrine in vivo were evaluated. Regional myocardial blood flows (RMBF) at tissue level were assessed by color-labeled microspheres at different time points. Results In VEH group, coronary vasodilatation response to Ach after 1 d of reperfusion was markedly reduced as compared to baseline, which still occurred after seven days of reperfusion, but SNP-induced vasodilatation was only slightly impaired and fully recovered seven days later. However, concentration-dependent responses to ACH in the coronary vascular bed were only partly preserved in DiOHF group after 1 d of reperfusion and completely restored after seven days of reperfusion. DiOHF also significantly improved the tissue level of reperfusion flow in the myocardium at risk and prevented the progressive reduction of RMBF which occurred in VEH group. Conclusion Bolus injection of DiOHF immediately before reperfusion significantly improves coronary endothelial function and the levels of regional myocardial blood flow in the border zone of myocardium at risk.
