摘 要:目的探讨64层螺旋CT冠状动脉成像在判别冠心病不同类型间斑块性质的方面的价值。资料与方法经冠状动脉造影证实明确诊断为冠心病的患者58例,其中稳定型心绞痛(SAP)组24例、不稳定型心绞痛(UAP)组34例,行64层螺旋CT冠状动脉成像,并对主要冠状动脉狭窄节段数、狭窄程度以及斑块性质进行比较。结果SAP组与UAP组患者冠状动脉狭窄节段数比较差异无统计学意义(P>0.05)。SAP组与UAP组在狭窄程度方面比较差异有统计学意义(P<0.01),UAP患者以中度狭窄为主(54.6%),SAP患者则以重度狭窄为主(52.0%)。两组斑块类型的分布差异有统计学意义(P<0.01),UAP组以软斑块为主(52.6%),混合斑块次之(35.8%);而SAP组则以钙化斑块为主(51.7%)。结论64层螺旋CT冠状动脉成像有助于显示斑块性质。Objective To investigate the application of coronary artery imaging with 64-slice spiral computed tomography in identifying plaque features among different type of coronary heart disease (CHD). Materials and Methods 58 patients with CHD confirmed by coronary angiography were divided into stable angina pectoris (SAP) group (n = 24 ) and unstable angina pectoris(UAP) group( n = 34). All patients underwent coronary artery imaging with 64 slice spiral computed tomography. The number and degree of stenosis and the plaque feature in the coronary artery were compared between two groups. Results There was no significant difference in the number of stenosis in the coronary artery between two groups. There was significant difference in the degree of stenosis in the coronary artery between two groups ( P 〈 0.01 ). Moderate stenosis of coronary arteries was detected in UAP group( 54.6% ) and severe stenosis of coronary arteries was detected in UAP group(52.0% ). Soft plaque was seen in UAP group (52.6%) and calcium plaque was found in SAP group (51.7% ). Conclusion Coronary artery imaging with 64 slice spiral computed tomography may be helpful in displaying the plaque feature of coronary'arteries in CHD patients.
