摘 要:目的:研究吡格列酮与胰岛素联合应用对缺血再灌注损伤心肌细胞的保护作用,探讨其作用机制。方法:取Wistar大鼠幼鼠心室肌细胞进行体外培养,建立缺血再灌注损伤细胞模型。应用台盼蓝排斥试验测定细胞存活率,比色法检测各组培养液中乳酸脱氢酶(LDH)、肌酸激酶(CK)、超氧化物歧化酶(SOD)和丙二醛(MDA)的含量,激光扫描共聚焦显微镜观察细胞内钙离子荧光强度,透射电镜观察心肌细胞超微结构。结果:吡格列酮与胰岛素联合应用可提高缺血再灌注损伤心肌细胞的存活率,降低细胞培养液中的LDH、CK和MDA含量,提高SOD的含量,降低心肌细胞钙离子染色的荧光强度,部分恢复心肌细胞的超微结构,与单独应用吡格列酮或胰岛素相比,具有统计学意义。结论:吡格列酮与胰岛素联合应用对心肌缺血再灌注损伤有更好的保护效果。Objective: To study the effect of pioglitazone and insulin on myocardial cell suffering ischemia reperfusion injury and to investigate its mechanisms. Methods: Newborn Wistar rats' hearts were isolated and cardiomyocytes were separated by trypsin digestion. The primary cardiomyocytes were collected and incubated in vitro. Ischemia reperfusion injury was established by 3 hours of hyopxia and 1 hour of reoxygenation subsquently. The cell survival rate, LDH,CK,SOD and MDA concentration and the flourescence stain of the intracellular calcium ion were detected by trypan blue exclusion, 722E visible range spectrophotometer and laser scanning confocal microscope respectively. The ultrastructure of myocardial cells was observed by transmission electron microscope. Results: Compared with I/R+insulin group and I/R+pioglitazone group, the ultrastructure of cardiomyocytes suffering ischemia reperfusion injury recovered markedly, the fluorescence intensity of the intracellular calcium ion decreased, the survival rate of cardiomyocytes increased, LDH, CK, MDA concentration in culture medium decreased and SOD concentration in culture medium increased in I/R+ insulin+ pioglitazone group. Conclusion: Compared with pioglitazone or insulin seperately, combination of them provides more preferable protection against ischemia reperfusion injury in the primary cultivated rat cardiomyocytes.
