摘  要:目的探索金标免疫渗滤法(DIGFA)快速检测患者血清中抗日本血吸虫IsM抗体的诊断和疗效考核价值。方法以虫卵可溶性抗原为抗原,羊抗人IgM(μ链)-胶体金结合物为检测标记物,创建DIGFA快速检测人血清中日本血吸虫特异性IgM的方法([grd.DIGFA);用IgM-DIGFA和IgG-DIGFA平行检测不同病期患者血清,评价其诊断和疗效考核价值。结果IgM-DIGFA检测急性和慢性血吸虫病患者的敏感性分别为100%(25/25)和95.8%(69/72),IgG-DIGFA的敏感性分别为100%(25/25)和97.2%(70/72),两法相比差异无统计学意义(P〉0.05);检测健康人血清,特异性分别为99%(99/100)和97%(97/100)。与IsG-DIGFA相比,IgM-DIGFA检测其他蠕虫病患者血清的交叉反应较低,仅与肺吸虫有3.3%交叉反应;IgM-DIG-FA检测治疗后6个月、12个月患者血清,阴转率分别为65.2%(15/23)和88.0%(22/25);IgG阴转率仅为21.7%(5/23)和64.0%(16/25)。结论DIGFA快速检测IgM的敏感性和IgG相近,且特异性更强,并具有潜在的疗效考核价值。Objective To evaluate the effect of diagnosis and treatment by detecting specific IgM of schistosomiasis japonica with dot inmauno-gold filtration assay (DIGFA). Method The DIGFA for detecting IgM of schistosomiasis (IgM-DIGFA) was developed by using soluble egg antigen (SEA) and sheep anti-human IgM immtmo-gold conjugate. Conventional DIGFA for detecting IgG (IgG-DIGFA) was applied as the parallel control for evaluating the effect of diagnosis and treaunent by detecting different kinds of sera. Results Detecting serum of acute and chronic schistosomiasis, the positive rate of IgM-DIGFA were 100% (25/25) and 95.8% (69/72) respectively, and the positive rates of IgC-DIGFA were 100% (25/25) and 97.2% (70/72) respectively. There are no statistical differences between two assays( P 〉 0.05). The specifieities of two assays were 99% and 100% respectively. The cross-reaction of IgM-DIG-FA was lower. Compared with IgG-DIGFA, only 3.3% specimen of patient infected wth paragonimus westermani was positive. The rates of negative turning of IgM-DIGFA in chronic schistosomiasis at the 6th month and 12th month intervals after treatment were 65.2% (15/23)and 88.0% (22/25)respectively. The rates of negative turning of IgG-DIGFA were 21.7%(5/23)and 64.0%(16/25) respectively. Conclusions It is sensitive and specific to detect IgM with DIGFA, as to detect IgC- IgM-DIGFA is also good for evaluating the treatment of schistosomiasis.
