阴道超声观测宫颈形态预测足月初产妇引产及分娩方式的研究

摘　　要：目的评价经阴道超声观测单胎头位足月初产孕妇后宫颈形态与引产潜伏期时间相关性及预测分娩方式的价值。方法219例需引产的单胎头位初产孕妇于引产前经阴道超声测量宫颈长度、宫颈管宽度，观察宫颈内口有无开大以及宫颈回声和位置，同时由专人用指检法进行Bishop评分，记录引产潜伏期、产程及分娩方式，评价宫颈形态与引产潜伏期及分娩方式的相关性。结果宫颈长度〉3．0cm者平均引产潜伏期和剖宫产率分别为（18．4±4．0）h和73．9％，宫颈长度≤3．0cm者平均引产潜伏期和剖宫产率分别为（7．9±2．0）h和27．7％，二者比较，差异有显著性（P〈0．01）。34例宫颈管轻度扩张（0．2～0．6cm）者32例引产成功，引产潜伏期（6±1）h。结论经阴道超声可以客观准确反映宫颈形态变化，经阴道超声测量宫颈长度能有效预测引产潜伏期及分娩方式，联合宫颈管宽度、宫颈回声等指标可用于评价初产妇引产前的宫颈成熟度。Objective To investigate the changes of cervix in induction of labor among the nulliparous by transvaginal uhrasonograph to evaluate the predictive effect of the cervical measurement. Methods The study recruited women with singleton gestation scheduled for induction of Iabor at≥37weeks. Transvaginal ultrasonographic cervical measurement was performed and Bishop score was determined. Data were collected on parity, gestational age, mode of delivery, induction agent, induction-onset of I abor-interval, Bishop score and cervical measurement. Results When the cervical length 〉 3cm, the induction to-onset of labor-interval was （ 18.4 ± 4.0 ） h and the cesarean section rate was 73.9% ;When the cervical length ≤ 3cm,the induction to-onset of 1 abor-interval was {7.9 ± 2.0 } h and the cesarean rate was 27. 7%. The difference was significant（ P 〈 0.01）. Those 34 nulliparous whose width of cervical tube（0.2 -0.6）cm, the induction to-onset of labor-interval was { 6 ± 1 ） h. Conclusions The changes of cervix are effective predictors of induction to-onset of labor-interval and the mode of delivery for the nulliparous. They can reflect the maturity of the cervix before the induction of the nulliparous.
